#OnThisDay in 1996, TAM Flight 402 crashes on takeoff from São Paulo, Brazil

20 years ago today, TAM Transportes Aéreos Regionais Flight 402 (PT-MRK) was a scheduled domestic flight from Congonhas-São Paulo International Airport in São Paulo, Brazil to Recife International Airport in Recife via Santos Dumont Airport in Rio de Janeiro. The Fokker 100 was carrying 90 passengers & 6 crew members.

ACCIDENT

At 8:25 TAM Flight 402 received clearance for takeoff from Runway 17R. Wind was given as 060 degrees. At 8:26 the throttles were advanced for takeoff power. Ten seconds later double beep was heard. The captain said “O auto-throttle” and the co-pilot adjusted the throttles manually and informed the captain: “thrust check”. With this information, he confirmed that the takeoff power had been adjusted and verified.

At 8:26:19 the aircraft accelerated through 80 kts. The co-pilot then indicated “V one”. Two seconds later the aircraft rotated at a speed of 136 kts. The aircraft was climbing at an angle of 10 degrees. At that same moment, a shock was felt and the EPR of engine no.2 dropped from 1.69 to 1.34, indicating the loss of power. In fact the no.2 engine thrust reverser had deployed.

The loss of power on the right side caused the plane to roll to the right. The captain applied left rudder and left aileron to counteract the movement of the plane. The co-pilot advanced both thrust levers, but they retarded again almost immediately, causing the power of the no.1 engine to drop to 1.328 EPR and engine no.2 to 1.133 EPR. Both crew members were preoccupied by the movement of the throttles and did not know that the thrust reverser on the no.2 engine had deployed. The throttles were forced forward again.

At 8:26:44 the captain ordered the autothrottle to be disengaged. One second later the no.2 thrust lever retarded again and remained at idle for two seconds. The airspeed fell to 126 kts. At 8:26:48 the co-pilot announced that he had disengaged the autothrottles. He then jammed the no.2 thrust lever fully forward again. Both engines now reached 1.724 EPR. With the thrust reverser deployed, the airspeed declined at 2 kts per second.

At 8:26:55 the stick shaker activated, warning of an impending stall. The aircraft rolled to a 39 degree bank angle and the GPWS activated: “Don’t sink!”. Seven seconds later the aircraft impacted a building and crashed into a heavily populated neighbourhood in São Paulo. All 96 on board were killed & 3 on the ground.

INVESTIGATION

The investigation into the fatal accident of TAM Transportes Aéreos Regionais Flight 402 was led by Brazil’s Aeronautical Accidents Investigation and Prevention Center & The National Transportation Safety Board (NTSB).

They concluded that a faulty mico-switch in conjunction with a possible short circuit caused the right engine’s thrust reverser to deploy. The imbalance of power resulted in the aircraft rolling and veering to the right as it was climbing away from the runway. The system automatically moved the thrust control to reduce power so the aircraft can climb out safety on the full power of the one unaffected engine there is no warning that the thrust reverser had deployed & the flight crew had not been trained for such an occurrence as the aircraft’s manufacturer, Fokker had judged the failure mode to be so remote a possibility that training for recovery was not necessary.

The safety system cable, responsible for pulling the power lever to the idle position, no longer able to withstand the physical strain of being pulled one way by the actuator, and being pulled the other way by the throttle lever being forced to the fully open position, parted at a maintenance connection. With the lever no longer restrained by the safety system, the co-pilot continued to hold the right throttle fully open. The combination of the right engine at full thrust in reverse and the left engine still at normal forward takeoff thrust, caused the aircraft to descend into the ground.

Eventful service over to Paris on the 25th of October! We boarded on time and were then informed around 10 minutes after taking our seats that there was a technical issue with the air conditioning unit which meant engineers needed to be called to our aircraft – we waited in a hot, stuffy cabin for around 1 hrs and 30 mins until the issue was finally resolved. A Russian fighter flew dangerously close to a US warplane over eastern Syria, US defence officials said Friday, highlighting the risks of a serious mishap in the increasingly crowded airspace.

The near miss occurred late on October 17, when a Russian jet that was escorting a larger spy plane manoeuvred in the vicinity of an American warplane, Air Force Lieutenant General Jeff Harrigian said. The Russian jet came to “inside of half a mile”, he added.

Another US military official, speaking on condition of anonymity , said the American pilot could feel the turbulence produced by the Russian jet’s engines.

“It was close enough you could feel the jet wash of the plane passing by,” the official said.

It appeared the Russian pilot had simply not seen the US jet, as it was dark and the planes were flying without lights. The American pilot tried unsuccessfully to reach the Russian jet via an emergency radio channel.

The next day, US officers used the hotline to ask Russia what had happened and they said “the pilot didn’t see” the American plane, the official said.

Aerolineas Argentinas flight #AR1300 from Buenos Aires to New York JFK suffered an engine issue during the flight.

The flight took off at 2357 (local time) when flames burst out of the Airbus A320 left engine 17 minutes later, forcing the pilots to divert to Ezeiza, Argentina.

The plane was met by fire engines, ambulances and rescue teams.

A 38-year-old pilot was arrested in South Dakota on Wednesday morning under suspicion of operating a flight under the influence of alcohol, local police say.

The pilot was taken into custody after TSA officers smelled alcohol on him and called local law enforcement, Rapid City Police Department says.

After an investigation, police said it was determined the pilot was in violation of the statute that prohibits flying with a blood-alcohol content of over .04.

The pilot from Salt Lake City, was operating Delta Connection 4574 flight to his home town. He is a pilot for SkyWest Airlines, a regional airline that was operating for Delta.

A plane carrying Republican vice presidential nominee Mike Pence skidded off a runway Thursday night as it landed at LaGuardia Airport in New York.
There were no reported injuries in the rough landing, which occurred on a rainy Thursday evening in the city.
The press pool in the back of the aircraft, a Boeing 737-700 chartered from Eastern Airlines that was painted with the campaign’s logo, could feel the plane fishtailing as it touched down and sliding off the runway before coming to a very sharp halt in the grass off the side of the runway.
Once the plane came to a full stop, the Indiana governor walked to the back of the plane to check on the press. As everyone on board deplaned, Pence could be seen speaking with emergency responders.
A spokesman for Pence said the vice presidential nominee would retire to his hotel for the evening, canceling an appearance at a closed-door fundraiser that had been scheduled in Trump Tower. He called into the event instead, a senior Pence aide said.

A senior official from Myanmar’s Civil Aviation Department in Yangon said “an unidentified aircraft” has crashed into the sea off the west coast on Friday (Oct 28) and authorities were trying to locate it.

“The only thing I can tell you for sure at the moment is that it is not an aircraft from our domestic airlines, so far as we’ve checked,” senior department official Tin Maung Ni told Reuters.

Seven million passengers per month is fast becoming the norm at Dubai International after September’s numbers eclipsed that total for the sixth time in nine months, according to the monthly traffic report issued today by operator Dubai Airports. Passenger traffic at DXB passengers reached 7,094,738 compared to 6,432,654 recorded in the corresponding month last year, an increase of 10.3 per cent.

KLM has been flying the Boeing 747 to St Maarten’s Princess Juliana Airport (SXM). The airport’s runway is known for its proximity to the beach, probably the most famous spot for plane watching.

But this is the end of the story: KLM will no longer be flying the Boeing 747 to St Maarten airport. The flight will be operated by an Airbus 330.

The search for the missing Malaysia Airlines flight MH370 is now predicted to continue into next year, approaching the third anniversary of the plane’s disappearance.

Bad weather continues to slow progress in the underwater search effort, coordinated by the Australian Transport Safety Bureau (ATSB) from Canberra. The 120,000 sq km area of the southern Indian ocean was expected to be completed by mid-2016, before being extended to December.

But the ATSB announced on Wednesday it would now be completed “by around January/February 2017”, with less than 10,000 sq km of the area to go.

It attributed this delay to poor weather conditions during the southern hemisphere’s winter. No ships are currently in action in the search zone, with the Fugro Equator having left to return to port on 15 October while the Dong Hai Jiu 101 remains berthed at Fremantle in Western Australia.

 

None of the “sonar contacts” so far resemble aircraft debris, but some have “man-made properties” and so warrant further investigation, the ATSB said.

NEWS Lufthansa flight makes safe landing in Gander after smoke appears in the cockpit.

A Lufthansa transatlantic flight was forced to make an unscheduled landing in Gander on Tuesday after the crew noticed smoke in the cockpit.

The Boeing 747-400 aircraft was on a transatlantic flight as LH464 when the pilot’s noticed smoke in the cockpit.

The flight’s departure was delayed by an hour for unknown reasons and eventually took off from Frankfurt at around 14:15 CEST.

When the aircraft was around 5 hours into the flight when the crew elected to make quick descent to 10,000 feet and divert to Gander as a safety precaution of smoke.

Due to an occurence of smoke within the cockpit the crew decided to divert th LH 464 to Gander. It touched down safely and Lufthansa plans to send another aircraft in order to get the passengers to their final destination Orlando.

The aircraft landed safely at around 17:00 NDT and haven’t been any reports of injuries.

NEWS Russian Woman Arrested at Athens Airport Carrying 2.3 million euros

Greek authorities on Friday arrested a Russian woman arriving at Athens International Airport from Moscow carrying with her about 2.3 million euros that were undeclared.

The incident occurred at 11.45 am on Friday at Greek customs upon the arrival of an Aeroflot flight from Moscow. When searched, customs officials found in her luggage and on her body 2.2135 million euros and 58,900 U.S. dollars in cash.

The money was hidden in special compartments in her luggage and part of it was inside her clothes in a latex bodysuit. The woman had not declared the money in Moscow and to Greek customs.

Customs made use of bill 1889/2005 for bringing undeclared money in the country which provides for a fine of 25% of the amount (553,375 euros). However, if after further investigation it is proven that the money was not obtained legally and it is a case of money laundering, then the whole amount will be confiscated.

ALERT Near collision between China Eastern Airlines A320 and A330 in Shanghai last Tuesday

According to the investigation, an Airbus A320 jet was taking off from a runway on Tuesday, as an Airbus A330 cut across his path at Shanghai Hongqiao International Airport.

The Captain of China Eastern Airlines A320 flight #MU5643 to Tianjin had been cleared for take-off by air traffic control, but an Airbus A330 was crossing the same runway after landing runway 36R. The captain then decided to continue with the take-off runway 36L.

Investigation shows the pilot barely managed to evade the crash, and his plane was just 19 meters above the taxiing aircraft. There were no injuries reported.

After the incident, the Civil Aviation Administration of China (CAAC) immediately founded a dedicated team to investigate the “serious incident”. The regulators have begun examining the cockpit voice recorder and flying data of both aircraft as well as talking with the pilots and the air traffic controller, the administration said.

The preliminary investigation by the CAAC has found that the serious incident was caused by wrong directions given by the control tower.

#OnThisDay in 1956, Pan Am Flight 6 ditches in the Pacific Ocean

60 years ago, Pan Am Flight 6 (N90943) was an around a world flight from Philadelphia to San Francisco with stops in Europe, Asia and the Pacific. At the time of the accident Pan Am Flight 6 was on the last leg of the flight from Honolulu to San Francisco. The Boeing 377 Stratocruiser was carrying 24 passengers & 7 crew members.

ACCIDENT

Pan Am Flight 6 took off from Honolulu International Airport, Hawaii at 20:26 HST for the last leg of the flight to San Francisco. The flight was cleared via Green Airway 9, track to 30 deg N, 140deg W at 13,000 ft and then at 21,000 ft to San Francisco. About half way, at 01:02 the crew requested a VFR climb to 21,000 ft, which was approved.

Immediately after reaching this altitude at 01:19 the no.1 engine oversped. Reduction of airspeed didn’t help and the prop could not be feathered, so the engine was cut. As the aircraft was losing altitude a ditching seemed imminent. US Coast Guard weather station ‘November’ was contacted at 01:22 about the possible ditching. Climb power was then applied to the remaining engines.

The no. 4 engine however, was only developing partial power at full throttle. Despite these problems the crew managed to maintain altitude at 5000 ft at an airspeed of 135 knots. Remaining fuel was insufficient to reach San Francisco or fly back to Honolulu. The crew decided to orbit US Coast Guard Cutter Pontchartrain and wait for daylight to carry out the ditching. Meanwhile electric water lights were laid by the cutter to illuminate a track for the aircraft.At 02:45 the no.4 engine backfired and failed. The prop was feathered. At 05:40 the captain contacted the cutter again about the intended ditching time and descended to 900 ft.

The ditching was carried out at 06:15 in the Pacific Ocean Northeast of Hawaii with full flaps, gear up and at a speed of 90 knots. The fuselage broke off aft of the main cable door. The tail section swung to the left, trapping the life-raft launched from the main cabin door. Some 3 minutes after all the occupants had been rescued. At 06:32 the aircraft sank.

The probable cause of the crash of Pan Am Flight 6 is an initial mechanical failure which precluded feathering the no. 1 propeller and a subsequent mechanical failure which resulted in a complete loss of power from the no. 4 engine, the effects of which necessitated a ditching.

Aeroflot plane is being checked by bomb disposal teams in Geneva, Switzerland, after a bomb threat was reportedly received.

Geneva Prosecutor’s Office reported the aircraft has been evacuated of all 115 passengers after the threat was received at the airline ticket counter.

Flight #SU2381 was bound for Moscow, Russia. A Russian man has reportedly been arrested and is now being questioned by police.

The passengers are waiting to be transferred to another plane to continue their journey while the bomb squad investigate the plane.

A female flight attendant had various sexual encounters with other crew members, including pilots in the cockpit, during work hours, according to a journal she kept at home, her husband claims.

He found the digital diary she kept about her extramarital affairs and is now threatening to publish the escapades in detail, the Telegraaf reports.”

The 46-year-old flight attendant had sex with various pilots and crew members during overnight stays at various locations,she writes in her diary, according to her husband. Some of the encounters happened on Transavia flights in the cockpit or crew area, according to the newspaper.

The woman filed a lawsuit against her husband in an effort to keep her escapades out of public eye. The court in The Hague will listen to the matter on October 24th, in a confidential hearing.

Transavia supports the woman in her case. The airline wants nothing more than to handle the matter internally. Especially as the diary does not only involve the flight attendant herself, but also a number of mostly married pilots, according to the Telegraaf.

Documents the Telegraaf got hold of through various sources show that the matter has been a problem for Transavia for months. Last month the airline’s lawyer told the husband to stop sending revealing emails to employees, threatening him with slander or libel charges. Before that the human resources department also warned him of criminal consequences that could come of him publishing the diary.

NEWS Investigations launched into “tail-strike” at Australia’s Melbourne Airport.

Investigations have been launched after a Singapore Airlines Boeing 777-300 was involved in a suspected “tail strike”.

Singapore Airlines flight SQ238 was departing Melbourne Airport’s runway 34 on Sunday morning when the aircraft’s tail came in contact with the runway.

Singapore Airlines have said the pilot’s did not receive any indications of a tail strike until Melbourne Tower (120.5) notified the pilots of sparks and possible smoke coming from the tail.

The Airport in Australia’s South East was closed for around 30 minutes and flights were diverted for engineers to inspect the runway for debris. During the inspection the workers found scrape marks on the runway which confirmed the tail strike.

The pilot’s decided not to return to Melbourne to get the aircraft inspected.

Upon arrival in Singapore the aircraft was inspected and was found that the tail skid system was damaged but the fuselage was undamaged.

The weather in Melbourne at the time was not favourble for pilot’s, wind speeds were gusting upto 119kmh from the North. 

All 282 passengers and crew on board were safely disembarked with no injuries.

Finland’s defence ministry said a Russian fighter jet violated Finnish airspace early last Thursday, after scrambled jets identified the SU-27 fighter over the Baltic Sea.

“The suspected violation… continued approximately one minute, and was in Finnish airspace for about 13 kilometers at a maximum of about one kilometer depth,” the ministry said in a statement.

A suspected radioactive leak from a medical shipment took place at the cargo terminal of Indira Gandhi International Airport on Sunday morning (Oct 9), leading authorities to cordon off the area.

The suspected radioactive substance came from an Air France aircraft.

Representatives from the Atomic Energy Regulatory Board and National Disaster Management Authority were present at the scene, said reports. Delhi Fire Services said it was alerted to the incident at 10.40am local time and dispatched at least seven fire engines to the cargo complex.

Air France confirmed later that the shipment of radioactive medical material on flight AF226 operated by an A330 from Paris CDG to New Delhi, on 8 October 2016, did not reveal any anomaly.

After several controls by the relevant authorities, including Indian’s independent Nuclear Energy authorities, at its arrival at the New Delhi airport its acceptation has been confirmed compliant. The transport of this type of material is subject to strict rules and carried out in accordance with the international regulations.

Air France reminds that the security of its customers and crews is its top priority,” the air carrier company said in its official statement.

Qatar Airways announced an order for 30 787-9 Dreamliners and 10 777-300ERs, valued at $11.7 billion at list prices, validating the value, reliability and performance of Boeing’s twin-aisle airplanes. The airline also signed a Letter of Intent for up to 60 737 MAX 8s, valued at $6.9 billion at list prices. Today’s announcement builds on Qatar Airways’ current fleet of 84 Boeing aircraft, a combination of 787s and 777s, all delivered over the last nine years. With this new order, Qatar Airways increases its firm order backlog of Boeing widebody airplanes from 65 to 105, including 60 777Xs.

EMERGENCY A twin-engine plane with six people on board lands with nose wheel up today

A twin-engine plane with six people has made an emergency landing at Lelystad Airport (Netherlands) Sunday afternoon  after problems with the landing gear.

The aircraft was on an approach to Eelde Airport when the pilots noticed that the front landing gear was not extended. They decided to return to their home base Lelystad. Meanwhile, the emergency services were on standby along the runway.

After a final visual check from police helicopter, the pilots did a low approach and landed safely.

A brand new Cessna CitationJet (D-IEVX) operated by Air Evex arrived at Linate Airport in Milan following a flight from Cologne Bonn Airport, Germany. The Cessna was to carry out a return flight to Paris-Le Bourget Airport, carrying two pilots, a Cessna sales manager and a prospective customer.

The plane arrived at 06:59 and was taxied to the General aviation apron, also known as ‘West apron’. It was a foggy morning at Milan and one of the passenger flights parked on the North apron was Scandinavian Airlines Flight 686 scheduled to depart at 07:35.

At 07:41, the pilot of the MD-87 contacted Linate Ground Control for his engine start clearance, as the boarding of 104 passengers had been completed. The Ground controller cleared the pilot to start engines and advised that the slot time for takeoff of the flight was at 08:16. Thirteen minutes later SAS Flight 686 was cleared to taxi to runway 36R: “Scandinavian 686 taxi to the holding position Cat III, QNH 1013 and please call me back entering the main taxiway.”

A few minutes later, the Cessna pilot requested permission to start the engines. The ground controller then gave start-up clearance. The ground controller then requested SAS Flight 686 to contact the Tower controller. From this moment on the crew of the MD-87 and the crew of the Cessna were tuned on two different radio frequencies. At 08:05 the pilots of the Cessna received taxi clearance: “Delta Victor Xray taxi north via Romeo 5, QNH 1013, call me back at the stop bar of the…. main runway extension.”

The pilot acknowledged by saying: “Roger via Romeo 5 and … 1013, and call you back before reaching main runway.” The Cessna started to taxi from the General Aviation parking position, following the yellow taxi line. After reaching the position where the yellow taxi line splits into two diverging directions, the pilot erroneously took the taxi line to the right and entered taxiway R6. At 08:09 The Ground controller cleared the Cessna to continue its taxi on the North apron. At the same time the Tower controller cleared SAS Flight 686 for takeoff: “Scandinavian 686 Linate, cleared for takeoff 36, the wind is clam report rolling, when airborne squawk ident.” The pilot advanced the throttles and acknowledged the clearance: “Clear for takeoff 36 at when… airborne squawk ident and we are rolling, Scandinavian 686.” When the MD-87 was speeding down the runway, the Cessna crossed the runway holding sign and entered the active runway 18L/36R.

At 08:10:21 the nose landing gear of the MD-87 had left the ground and main gears were extending the shock absorbers but the main wheels were still on the ground at an airspeed of 146 knots. At the moment the MD-87 crew probably saw a glimpse of the Cessna through the fog and reacted with additional large nose-up elevator. It collided with the CitationJet. The right wing of the MD-87 sustained damage at the leading edge and the right hand main landing gear leg broke off. It damaged the right flap and struck the no.2 engine which then separated from the pylon. The pilot of the MD-87 gradually advanced the throttles and then the aircraft was airborne for a total of 12 seconds, reaching an estimate height of about 35 ft. The left hand engine suffered a noticeable thrust reduction as a result of debris ingestion which became insufficient to sustain flight.

The airspeed had increased up to 166 knots, but the MD-87 descended abruptly making contact with the runway with the left hand main gear, the truncated right hand main gear leg and the tip of the right hand wing. Prior to touch down the pilot reduced engine thrust and after ground contact the engine reverse levers were activated and deployed on the left hand engine. Maximum available reverse thrust was selected and the brakes applied. The plane skidded past the grass overrun area, across a service road, crashing sideways into a baggage handling building, which partly collapsed. In total 118 people lost their lives. All on both aircraft and 4 on the ground.


INVESTIGATION 

The investigation into the cause of this accident was led by the ANSV (Italy National Agency for Air Safety).

They concluded that the immediate cause for the accident has been the runway incursion in the active runway by the Cessna. The obvious consideration is that the human factor related action of the Cessna crew – during low visibility conditions – must be weighted against the scenario that allowed the course of events that led to the fatal collision; equally it can be stated that the system in place at Milan Linate airport was not geared to trap misunderstandings, let alone inadequate procedures, blatant human errors and faulty airport layout.

EMERGENCY Overheated Samsung phone causes Southwest flight #WN994 evacuation

A Southwest airplane was evacuated at the Louisville International Airport on Oct. 5.

An airport spokesperson said smoke was reported in the airplane’s cabin at 9:15 a.m. and no one was injured.

Louisville Metro Arson confirms a Samsung phone was the cause of the smoke. Officials said the phone overheated.

The phone had some heat damage, but also singed the carpet of the plane. The Boeing 737 Flight #WN994 to Baltimore was at the gate when this happened.

The passenger said he was powering his Samsung Galaxy Note7 down when it started smoking. He said it was a refurbished phone that was deemed safe.

NEWS Southwest flight returns to gate to inform a passenger her son is in coma

Southwest Airlines is getting a lot of positive attention after they did something truly awesome for one of their passengers when they learned her son was in a coma.

Peggy Uhle was already settled in her seat on a flight from Chicago, Illinois, to Columbus, Ohio. The plane was ready for take-off, but it suddenly turned back towards the gate. That’s when a flight attendant approached her and informed her that she needed to get off the plane.

“I figured I was on the wrong plane,” said Peggy. “The gate agent told me to check in at the desk and when I did she told me to call my husband.” That’s when she learned from her husband that her son, who lives in Denver, Colorado, had suffered a head injury and was in a coma.

Before she even got off the plane, Southwest Airlines had already made her arrangements for her. “The gate attendant already knew the situation and had booked me on a direct flight to Denver that was leaving in the next two hours,” said Peggy. The airline also didn’t charge Peggy any additional service fees.

It’s the third time in a month this Boeing 757 has technical issue. In September, this aircraft returned two times to Manchester after landing gear failed to retract.

#OnThisDay in 1996, Aeroperú Flight 603 crashes into the ocean off Pasamayo, Peru

20 years ago today, Aeroperú Flight 603 (N52AW) was a scheduled flight from Miami International Airport, Florida to Comodoro Arturo Merino Benítez International Airport, Chile with stopovers in Ecuador and Peru. The Boeing 757-23A was carrying 61 passengers & 9 crew members.

The cockpit crew consisted of Captain Eric Schreiber (Left), 58.  Schreiber was a veteran pilot who had logged almost 22,000  flight hours. The First Officer was David Fernández (Right), 42. Fernández had logged almost 8,000 flight hours. Both were very experienced pilots.

On 1 October, Aeroperú Flight 603 from Miami International Airport had landed at Lima Airport using a Boeing 727-200 because of technical problems with the Boeing 757 that normally flew this route. 180 passengers were on the first leg of the flight. 119 had disembarked and the remaining passengers were transferred to the Boeing 757 after maintenance checks.

On 2 October at 12:42am Aeroperú Flight 603 took off from Lima-Jorge Chavez International Airport, Peru for a flight to Santiago-Arturo Merino Benitez Airport, Chile. Five minutes after takeoff the crew reported problems with their instruments and stated they wanted to return to the airport.

During the initial climb, the airspeed and altitude indications were too low. in calm winds, the windshear warning suddenly sounded. The aircraft climbed to FL130, before declaring an emergency and return to Lima was initiated. While returning the captain’s airspeed and altitude indications were too high, causing an overspeed warning. At the same time, the co-pilots airspeed indications were too low, triggering the stick shaker.

The air traffic controller had instructed a Boeing 707 to takeoff to help guide the 757 back in to land, but before the 707 could take off, the 757’s wingtip struck the water, approximately 25 minutes after the emergency declaring an emergency. The aircraft pulled up at about 200ft and crashed at a speed of 260 knots inverted. The captain’s airspeed indicated 450 knots and an altitude of 9500 ft. All 61 passengers & 9 crew members were killed.


INVESTIGATION

The investigation into the cause of the crash of Aeroperú Flight 603 was led by the Commission of Accident Investigations (CAI) & the National Transportation Safety Board (NTSB).

They concluded that the probable cause was the error of the maintenance staff including the crew. It can be deduced from the investigation carried out that the maintenance staff did not remove the protective adhesive tape from the static ports. The tape was applied before washing and polishing the aircraft prior to the accident flight. This tape was not detected during the various phrases of the aircraft’s release to the line mechanic.

It transferred to the passenger boarding apron and lastly the inspection by the crew responsible for the flight (the walk around or pre-flight check), which was carried out by the pilot-in-command according to the mechanic responsible for the aircraft on the day of the accident.

The pilot-in-command made a personal error by not complying with the procedure for GPWS alarms and not noticing the readings of the radio altimeters in order to discard everything which he believed to be fictitious. The co-pilot made a personal error by not being more insistent, assertive and convincing in alerting the pilot-in-command much more emphatically to the ground proximity alarms.
