30 years ago today, Aeroméxico Flight 498 (XA-JED) was a scheduled passenger flight from Mexico City International Airport, Mexico to Los Angeles International Airport, USA with stopovers in Guadalajara, Mexico; Baja California Sur and Tijuana, Baja California. The McDonnell Douglas DC-9-32 was carrying 58 passengers & 6 crew members at the time of the accident.

The cockpit crew of Flight 498 consisted of Captain Arturo Valdes Prom, age 46 (pictured) and First Officer Jose Hector Valencia, age 26. The captain had 4,632 hours flying experience in the DC-9 and a total of 10,641 flight hours. The first officer had flown 1,463 hours in total, of which 1,245 hours had been accumulated in the DC-9.

Aeroméxico Flight 498 departed from its last stopover at 11:20am and proceeded to Los Angeles at FL100.

At about 11:44AM, Coast Approach Control cleared Flight 498 to 7000ft to began their initial descent into LAX. Just three minutes earlier Piper PA-28-181 Cheorokee N4891F departed Torrance Airport, California for a VFR flight to Big Bear, California.

On board were a pilot & two passengers. The Piper pilot turned to an easterly heading towards Paradise VORTAC and entered the Terminal Control Area (TCA) without receiving clearance from ATC as required by FAR Part 91.90.

At 11:47 the Aeroméxico pilot contacted LA Approach Control and reported level at 7,000ft. The approach controller cleared flight 498 to depart Seal Beach on a heading of 320 degrees for the ILS runway.

At 11:51:04 the approach controller asked the flight to reduce its airspeed to 190 knots and cleared it to descend to 6000ft.

At about 11:52:09 flight 498 and the Piper collided over Cerritos, Los Angeles County at an altitude of about 6560 feet. The piper struck the left hand side of the DC-9 horizontal and vertical stabilizer. The horizontal stabilizer sliced through the Piper’s cabin following which it separated from the tailplane. Both planes tumbled down out of control.

The wreckage and post impact fires destroyed five houses and damaged seven others. 15 people on the ground were killed and everyone on board both aircraft. In total 82 people lost their lives that day.


INVESTIGATION

The investigation into this accident was led by the National Transportation Safety Board (NTSB). They concluded that:

The Piper had entered Los Angeles Terminal Control without the required clearance. The Piper was not equipped with a Mode C transponder, which would have indicated its altitude and LAX had not been equipped with automatic warning systems. Neither attempted any evasive maneuvers because neither pilot sighted each other even though they were in visual range.

As a result of this accident and other near mid-air collisions in terminal control areas, the Federal Aviation Administration required all jets in US airspace to be equipped with a Traffic Collision Avoidance System (TCAS) and required that light aircraft operating in dense airspaces be equipped with Mode C transponders which can report their altitude.


MEMORIAL

On March 11, 2006, the City of Cerritos dedicated a new sculpture garden featuring a memorial to the victims of the accident. The sculpture, designed by Kathleen Caricof, consists of three pieces. One piece, which resembles a wing, commemorates all the victims who perished aboard the Aeroméxico jet and the Piper. A similar, but smaller and darker wing, commemorates all the victims who perished on the ground. Each wing rests on a pedestal that lists the respective victims in alphabetical order. In front of the memorial lies a bench, which commemorates all the victims and allows visitors to sit and reflect on the disaster.

United Airlines flight #UA880 from Houston to London made an emergency landing at Shannon following turbulence over Atlantic during the night.

The Boeing 767-300 (reg. N657UA) with 207 passengers and crew of 13 on board was flying over the Atlantic when it encountered severe turbulence.

The pilots decided to divert to Shannon in Ireland at 5:55am where it was met at the terminal by ambulance paramedics.

A spokesman confirmed “14 customers and 2 flight attendants have been taken to a local hospital. We wish these passengers and crew a quick recovery from their injuries”.

NEWS Passenger forces Alaska Boeing 737 into emergency landing after trying to open door mid-flight

A man reportedly attempted to open an exterior airplane door during a packed cross-country flight Monday, prompting an emergency landing at Minneapolis-St. Paul International Airport.

Alaska Airlines flight 7, traveling from New York City to Seattle, made an emergency landing at the Twin Cities airport about 9:45 a.m.

Once on the ground, an “unruly passenger” who, according to flight crew, had attempted to open an exterior door on the plane was removed by law enforcement.

An airport spokeswoman confirmed that a man was removed from the flight by police and taken to a nearby hospital, and that the investigation would be taken over by the FBI.

Delta #DL219 Boeing 767 to JFK returned to Copenhagen for an immediate landing with 217 passengers and crew on board.

A fireball from the left engine has be seen during take off. The tower immediately informed the pilots of the suspicion was that the plane had hit a bird, a so-called ‘birdstrikes’. The Boeing 767 safely landed few minutes later and Copenhagen Airport announced engineers have found traces of a bird in the engine, and that probably is the technical fault on the engine.

NEWS “Zorro” causes evacuation of Los Angeles Airport

Part of Los Angeles International Airport was briefly closed late Sunday (Monday European time) after loud noises were heard that sounded like gunfire, possibly caused by a man dressed in a black Zorro outfit who was later detained by police.

Passengers who fled in panic after hearing the noises slowly filtered back to the busy airport, as the departure and arrival areas of the central terminal that were closed for a police sweep were re-opened.

“Report of shooting at LAX proven to be LOUD NOISES only,” Los Angeles Police Department chief spokesman Andy Neiman wrote on Twitter.

“No Shots Fired No Injuries investigation continues to locate source,” he added.

However an individual “in Zorro costume has been detained” by airport police, airport authorities wrote on Twitter. It was not immediately known what role the individual played in the incident.

Following the reports of an active shooter, passengers “in several LAX terminals self-evacuated onto the tarmac and rushed through federal security screening without being properly screened,” airport public relations director Nancy Suey Castles said after the terminal was cleared.

Many passengers however were “being screened by explosive detection K-9 units” before being allowed to enter the terminals for re-screening, she said, adding that reports “of an active shooter at LAX were unfounded.”

Qatar’s main Hamad International Airport is adding a $10 fee for all passengers departing from the airport, including on connection flights.

The decision, announced on Sunday, comes after the Middle East’s busiest airport in Dubai added similar fees earlier this year.

The measure helps boost revenue in Gulf countries following a slump in the price of oil that has impacted their economic growth and limited spending capabilities.

The fee will be added to tickets for travel starting Dec. 1. Children under two-years-old without a seat will be exempted.

NEWS Mechanical Issue Forces Diversion of Delta Airlines flight.


A delta flight inbound to Atlanta made a diversion to Orlando International Airport after a flap mechanical issue occurred shortly after take-off according to OIA officials.

Delta Airlines flight DL1827 from Fort Lauderdale to Atlanta landed safely in Orlando at 15:05 EDT and no injuries were reported.

As a safety precaution, the Aviation Rescue Fire Fighting (ARFF) was deployed into the area.

Passengers have since been transported to Atlanta on a different aircraft which departed Orlando at 17:51 EDT.

The drama unfolded over claims the two men had been drinking and were too sozzled to fly the Boeing 757-200 (reg. N34137).

They had been due to be part of a three-man crew on the flight deck during the transatlantic journey. It’s understood the plane’s captain was not involved in the incident.

The pilots were arrested by cops and will appear at Paisley Sheriff Court tomorrow.

Last night a Police Scotland spokesman said: “We can confirm that two men aged 35 and 45 have been arrested and are presently detained in police custody in connection with alleged offences under the Railways and Transport Safety Act.”

10 years ago today, Comair Flight 5191 (N431CA) was a domestic passenger flight from Blue Grass Airport, Lexington, Kentucky to Hartsfield-Jackson Altanta International Airport, Georgia. The Bombardier CRJ-100ER jet was carrying 47 passengers & 3 crew members.

The cockpit crew of Comair Flight 5191 were Captain Jeffrey Clay (left) and First Officer James Polehinke (right). At the time of the accident the first officer was the pilot flying the plane.

On the morning of 27 August at 06:00, Comair Flight 5191 taxied out for takeoff. The flight was cleared for a runway 22 departure, which is Lexington’s main runway. Because the runway was repaved just recently with an added safety area at the approach end of runway 22, one taxiway was no longer in use. Since August 20 the taxi route to runway 22 was changed.

After confirming “Runway two-two”, Captain Jeffrey Clay taxied onto Runway 26, an unlit secondary runway only 3,500 feet (1,000 m) long, and turned the controls over to First Officer James Polehinke for takeoff.

The air traffic controller was not required to maintain visual contact with the aircraft; after clearing the aircraft for takeoff, he turned to perform administrative duties and did not see the aircraft taxi to the runway. As the First Officer commenced the takeoff roll. One of the pilots made a remark about the lack of runway light illumination, but the takeoff was continued.

Clay called for rotation but the aircraft sped off the end of the runway before it could lift off. It then struck a low earthen wall adjacent to a ditch, becoming momentarily airborne, clipped the airport perimeter fence with its landing gear, and smashed into trees, separating the fuselage and flight deck from the tail.

The aircraft struck the ground about 1,000 feet (300 m) from the end of the runway. Forty-nine of the 50 people on board perished in the accident; most of them died instantly in the initial impact. The resulting fire destroyed the aircraft.

The sole survivor was First Officer James Polehinke. He suffered serious injuries, including multiple broken bones, a collapsed lung, and severe bleeding. Polehinke underwent surgery for his injuries including an amputation of his left leg. Doctors later determined that Polehinke had suffered brain damage and has no memory of the crash or the events leading up to it. In August 2007, Polehinke filed a lawsuit against the airport and the company that designed the runway and taxi lights. The families of 21 of the 47 passengers filed lawsuits against Polehinke.


INVESTIGATION

The investigation was led by the National Transportation Safety Board (NTSB) they concluded that the likely cause of the crash was pilot error.

They concluded that Clay and Polehinke ignored clues that they were on the wrong runway, failed to confirm their position on the runway and talked too much, in violation of “sterile cockpit” procedures. Comair later accepted responsibility for the crash, but also placed blame on the airport, for what it called poor runway signs and markings, and the Federal Aviation Administration (FAA) which had only one air traffic controller on duty contray to a memo it had previously issued to have two workers on overnight shifts. A judge ruled that, since it was owned by county governments, the airport had sovereign immunity and could not be sued by Comair.

Based upon an estimated takeoff weight of 49,087 pounds (22,265 kg), the manufacturer calculated a speed of 138 knots (159mph) and a distance of 3,744 ft would have been needed for rotation with more runway needed to achieve lift-off. At a speed approaching 100 knots (120mph), Polehinke remarked “That is weird with no lights” referring to the lack of lighting on Runway 26. “Yeah”, confirmed Clay, but the FDR gave no indication either pilot tried to abort the takeoff as the aircraft accelerated to 137 knots (158mph).

An easyJet flight from London’s Gatwick Airport to Belfast has faced an hourlong delay after two crew members got into an epic shouting match.

Shocked passengers tweeted about the argument that grew so heated that the two crew members had to be removed from Wednesday’s flight.

Dan Lobb, a television presenter on the flight, described the scene as “unreal.”

He tweeted: “This is quite incredible. We’ve all worked with people we don’t get on with right? But this tiff means a 1 hr flight is delayed!”

The low-cost carrier confirmed what it called a “verbal disagreement” and apologized to passengers for the inconvenience.

The company said Thursday that the safety and welfare of EasyJet passengers and employees required crew members “to be able to work as a team.”

NEWS ANA cancels Dreamliner flights due to engine problems

Japanese carrier ANA said on Thursday it needs to replace damaged compressor blades in the Rolls-Royce engines powering its Boeing 787s, forcing it to cancel some Dreamliners flights over the coming weeks.

The carrier said that under certain flying conditions the compressor blades in the engine’s interior showed corrosion. It said it will cancel nine domestic 787 flights on Friday, with other cancellations likely in the coming weeks.

ANA’s Dreamliners are powered by Rolls-Royce Trent 1000 engines. The Japanese airline is the biggest operator of the Boeing model with some 49 aircraft.

The photos, which were believed to have been taken about a year ago, were reportedly shared to Facebook and Instagram after staff sent them to Snapchat.

A spokeswoman for Air New Zealand told News Corp the images were only brought to the airline’s attention “in the past 48 hours”. She said an investigation had “commenced immediately” when staff were alerted.

One of the staff members concerned no longer works for Air New Zealand and the other two have been removed from duties pending the outcome of the investigation, according to the spokeswoman.

Security threat on flight Egyptair Airbus A330 (reg. SU-GDS) flight number MS799 scheduled to leave Cairo at 0935 LT to Paris CDG.

Because of an unidentified bag, the plane has been moved to an isolated area for inspection. Passengers will also be re-screened.

ALERT Gunshot fired on board Vueling A320 flight #VY3420 to Madrid

A gunshot was accidentally fired on board Vueling Airbus A320 aircraft. Fortunately the incident caused no injuries, although the sound of the shot and the smell of gunpowder sparked unrest among the passengers.

The incident occurred at Ibiza airport, when flight VY3420 was about to take off to Madrid Barajas. Four agents of national police were traveling on the flight and as mark the protocol in such cases, the captain requested them to give up their weapons to be able to keep them in the cabin until they reached Madrid.

To prevent passengers to see the weapons, police turned away to one side of the row and manipulated the weapons inside a suitcase. However, one shot was fired but the bullet never came out of the suitcase.

Engineers were required to verify if the aircraft had not suffered any damage. Finally, the plane left to destination 30 minutes late.

#OnThisDay in 2015, A vintage jet crashes during a display at the Shoreham Airshow

1 year ago today, a Canfield Hunter Limited Hawker Hunter T7 (G-BXFI) was taking part in the first of the two-day Shoreham Airshow, held in aid of the Royal Air Forces Association. The Hunter had been opening the afternoon session of displays.

The pilot of the vintage jet was Andy Hill, 51. He was described by colleagues as experienced, with more than 12,000 flights hours. He had worked as a captain at British Airways. He had flown Harrier Jump Jets and worked as an instructor for te RAF before joining the airline. As well the Hawker Hunter, he flew Van’s Aircraft RV-8 and a BAC Jet Provost at airshows.

The Hunter commenced its display with a low pass along the runway from south to north, turning for a second pass in the opposite direction. As it neared the airport, it pulled up into an inside loop. This manoeuvre started from a height of 200 ft, which David Learmount later said “left no room for misjudgement”. Before it could complete the loop the aircraft crashed in a nose-high attitude onto the west bound carriageway of the A27 road.

The aircraft broke into four main parts on impact destroying several cars at 13:22 BST (12:22 UTC)

Eleven people on the ground were killed, and 16 others were injured. Those confirmed dead included two players from Worthing United F.C. English football team. Eight vehicles were destroyed, including a Daimler DS420 limousine which was en route to collect a bride to transport her to church for her wedding. The driver of the Daimler was subsequently confirmed one of the victims.

The pilot was thrown clear of the aircraft in his ejection seat, which was live when the aircraft departed from North Weald. He survived the crash with serious injuries. He was flown to the Royal Sussex County Hospital nearby Brighton; his condition was described as critical and he was said to be fighting for his life. He was subsequently placed in a medically-induced coma. He was released from hospital in September 2015.


INVESTIGATION

The Air Accident Investigation Branch (AAIB) who is responsible for investigating civil aviation accidents and incidents in the United Kingdom, sent a team to Shoreham. As part of the investigation, the AAIB appealed for members of the public to contact them if they had photographs or video of the accident, and received a large number of such recordings from variety of locations around the airport.

The AAIB published an interim report on 4 September. The report stated that “To date, no abnormal indications have been indications have been identified. Throughout the flight, the aircraft appeared to be responding to the pilots control inputs”.

A second special bulletin was published by the AAIB on 21 December. It revealed that the aircraft was not compliant with its Permit to Fly insofar as the ejection seat cartridges installed had passed their expiry date.

A third special bulletin was published on 10 March 2016. It covered the organisation of the airshow at Shoreham, and also the United Kingdom in general, with particular attention to risk management. It reported that at the 2014 Shoreham airshow the aircraft G-BFXI had overflown lancing with a bank angle in excess of 90°. The pilot had not been instructed to stop his display. It was also reported that the Flying Display Director at both displays did not have prior knowledge of the display intended to be flown.

The investigation is still undergoing.

Mariah Carey will appear in a series of concerts at the Colosseum at Caesars Palace in Las Vegas this month but you probably won’t see any ads for her shows at Bob Hope Airport in Burbank.

The Airport Authority, which runs the facility, decided last week that a proposed ad for the top-selling singer showed a little too much skin to be posted at the airport.

The government body that runs the airport said the ads needed “more coverage” of Carey before they could be posted at Bob Hope Airport, said airport spokeswoman, adding that the concert promoter has yet to respond with a more modest ad.

Sri Lanka’s national carrier has suspended a captain who failed a breathalyser test just before he was due to fly 274 passengers and crew from Frankfurt to Colombo, the airline said on Sunday.

An airline source, speaking on condition of anonymity, said Flight UL554 was held up at Frankfurt airport for more than 15 hours on Friday as staff scrambled to find another captain for the Airbus A330.

The crew raised the alarm after noticing that the captain was visibly drunk, the source, who has direct knowledge of the matter, told AFP.

Low-cost airline Norwegian continues its UK expansion, by putting on sale two new routes from London Gatwick to Keflavik, Iceland and Rovaniemi, Finland.
