HISTORY 1 year since Virgin Atlantic Flight #VS43 landing scare

1 year ago today, Virgin Atlantic Flight #VS43 (G-VROM) with 447 passengers & 18 crew members was a scheduled passenger flight from London Gatwick to Las Vegas. The aircraft took off at 11:43GMT, shortly after the pilots discovered a problem with part of its main landing gear.

The Boeing 747 circled over southern England for several hours dumping fuel before making a safe landing back at Gatwick shortly before 16:00GMT.

Route took by Virgin Atlantic Flight VS43.

It was later found out that a maintenance personnel installed the main landing gear actuator upside-down before the aircraft suffered an undercarriage jam while returning to London Gatwick. The maintenance team did not use a sling to hold the 85kg actuator in place as it was fitted. Two technicians instead manually supported it - holding the heavy component for over 30 mins, owing to delays in locating an attachment pin.

Incorrect installation resulted in forces that distorted a hydraulic port during retraction, causing the fluid leak and subsequent gear jam.

QZ8501 took off from Juanda International Airport at 05:35 Western Indonesian Time 28 December 2014. (22:35 UTC 27 December 2014). At 06:12WIT (23:12UTC) the pilot request permission to climb to 38,000ft to avoid big storm clouds. Shortly after at 6:18 Western Indonesian Time (23:18 UTC) ATC lost contact with the aircraft.

Two days after the crash, debris from the aircraft & human remains were found floating in the Java Sea. Searchers located wreckage on the sea floor on 3 January, and the black boxes were recovered on 13 January. By the end of March 2015, 113 of the 162 bodies were recovered.

On 20 January 2015, it was reported that the aircraft had stalled in bad weather during an abnormally steep climb and had been unable to recover.

FDR data at the time of the stall showed sidesticks dual input. The captain pulled up (LEFT). The co-pilot pushed down (RIGHT). Which resulted in no movement of the elevator control surfaces (=0) and the plane stalled until the recording ended.

On 1 December 2015, the NTSC released its report concluding that the sequence of events leading to the crash started with a malfunction in the rudder travel limiter unit that eventually led to a 104-degree roll of the aircraft. The pilots response & miscommunication was linked in the chain of events that led to the loss of the aircraft.

HISTORY In 1991, All 129 survived a crash landing near Gottröra, Sweden.

24 years ago today, Scandinavian Airlines Flight 751 (OY-KHO) was a scheduled passenger flight from Stockholm, Sweden to Warsaw Poland, via Copenhagen Denmark. The aircraft was piloted by Danish captain Stefan G. Rasmussen and Swedish first officer Ulf Cedermark.

Before take off, ice had collected on the wings close to the fuselage, which broke off and was sucked into the engines as the aircraft became airborne. Both engine's began to surge & the captain responded by throttling down a little, without the pilots knowledge it changed to automatic mode (ATR) which increased throttle setting with altitude. The high load from the surges caused the engines to break up & fail.

An off-duty captain, Per Holmberg entered the cockpit & assisted in troubleshooting, draft resolutions and strike planning. Per Holmberg repeated to the captain 'Look Straight Ahead' at least 20 times during the descent. The aircraft made a forced emergency landing in a field near Gottröra, Sweden. All 129 passengers & crew on board survived.

The investigation commission concluded that Holmberg effort "probably saved everyone on board & from an even worst crash"

Search is now the largest and most expensive in aviation history. Malaysia has spent £50m on the search and is still yet to be found.

Time is now running out! The search is expected to conclude in June, six months time.

In 2015, experts said they believe investigators are 'close to finding' the 777 which is most likely to have crashed in the sea.

Over 30,000 sq miles of sea floor has been searched with no trace of the plane.
