#OnThisDay in 1956, two planes collide over the Grand Canyon
60 years ago today, United Airlines Flight 718 (N6324C) was a scheduled flight from Los Angeles International Airport to Chicago Midway Airport. The Douglas DC-7 Mainliner was carrying 53 passengers & 5 crew members.

The pilots of United Airlines Flight were Captain Robert Shirely, First Officer Robert Harms and Flight Engineer Gerard Fiore.
The other aircraft involved in the collision was Trans World Airlines (TWA) Flight 2 (N6902C) on a scheduled flight from Los Angeles International Airport to Kansas City Downtown Airport. The Lockheed L-1049 Super Constellation was carrying 64 passengers & 6 crew members.

The pilots of TWA Flight 2 were Captain Jack Gandy, First Officer James Ritner and Flight Engineer Forrest Breyfogle.
At 9:01 a.m PST TWA Flight 2 took off from Los Angeles for Kansas City. After reporting on top of the clouds at 2400 ft the crew contacted Los Angeles ARTCC and clearance was given to climb to 19,000ft. The crew immediately asked for a routing change to Daggett via Victor Airway 210. This was approved.
At 9:21 a.m PST Flight 2 reported that it was approaching Daggett and requested a change in flight plan altitude from 19,000ft to 21,000ft. The request was denied because of traffic at FL210 (United Airlines Flight 718). A request for 1000ft on top was granted.
At 9:59 a.m PST Flight 2 reported its position through company radio at Las Vegas. It reported that it had passed Lake Mohave at 9:55, was 1,000 on top at 21,000ft, and estimated it would reach the 321-degree radial of the Winslow omni range station (Painted Desert) at 10:31. This was the last radio communication with this flight.
United Airlines Flight 718 had departed from Los Angeles at 9:04 a.m PST for Chicago cruising at FL210. At approximately 9:58 a.m Flight 718 made a position report to the CAA communications station in Needles. This report stated that the flight was over Needles at 9:58, at 21,000ft and estimated crossing the painted desert line at 10:31 a.m.
As the two aircraft approached the Grand Canyon, both aircraft were flying at the same altitude and nearly the same speed, the United Flight 718 was most likely manoeuvring around towering cumulus clouds. It is believed the planes simultaneously passed the same cloud formation on opposite sides, setting the stage for the collision.
At about 10:30 a.m PST the flight paths of the two aircraft intersected over the canyon. United 718 was heading 46 degrees magnetic and TWA 2 was heading 59 degrees magnetic. Both aircraft collided in mid-air.
First contact involved the centre fin leading edge of the Constellation and the left aileron tip of the DC-7. The lower surface of the DC-7 left wing struck the upper aft fuselage of the L-1049 with disintegrating force.
The collision ripped open the fuselage of the Constellation from just forward of its tail to near the main cabin door. The empennage of the L-1049 separated almost immediately. The plane pitched down and fell to the ground. Most of the left outer wing of the DC-7 had separated and aileron control was restricted, causing the plane to go out of control. The DC-7 collided with the south side cliff of Chuar Butte. All 128 perished in the crash.
Investigators concluded that the probable cause of this accident was the pilots did not see each other in time to avoid the collision. This resulted in one or a combination of factors: 1) Intervening clouds reducing time for visual separation; 2) Visual limitation due to cockpit visibility; 3) Preoccupation with normal cockpit duties; 4) Preoccupation with matters unrelated to cockpit duties such as attempting to provide the passengers with a more scenic view of the Grand Canyon area; 5) Physiological limits to human vision reducing the time opportunity to see and avoid the other aircraft, or; 6) Insufficient of en-route air traffic advisory information due to inadequacy of facilities and lack of personnel in air traffic control.
This accident spurred a $250 million update of the air traffic control (ATC) system. The crash also triggered the creation in 1958 of the Federal Aviation Agency FAA (now Administration) to oversee air safety.

NEWS Singapore Airlines Flight #SQ368 Catches Fire On Changi Airport Runway
A Singapore Airlines flight has been forced to make an emergency landing in Singapore after an engine issue.
Flight SQ368 with 222 passengers and 19 crew on board, departed Changi Airport Terminal 3 for Milan, Italy, at around 2.00 am on Monday.

The flight was 3 hours into the flight when the crew elected to return to Changi as a safety precaution.
According to a passenger, the aircraft’s right wing caught fire shortly after landing at 6:45am.
The airport’s fire services were quick on the scene to put out the fire and all 222 passengers were disembarked by stairs.
No injuries have yet been reported and the aircraft has been towed away from the runway.
Singapore Airlines released a statement on the incident.
Flight SQ368, a Boeing 777-300ER operating from Singapore to Milan, returned to Singapore on 27 June following an engine oil warning message.
The aircraft’s right engine caught fire after the aircraft touched down at Changi Airport at around 6:50am. The fire was put out by airport emergency services and there were no injuries to the 222 passengers and 19 crew on board.
Passengers disembarked through stairs and were transported to the terminal building by bus. Passengers will be transferred to another aircraft which is expected to depart for Milan later today.
Singapore Airlines will be co-operating fully with the authorities in their investigations.
Singapore Airlines says they are expected the passengers to on there way again later today.

A man had a unique experience on a Delta flight Tuesday. He was the only passenger on the plane.
Steven Schneider, of Lawrenceville, Ga., told WSB-TV he arrived at the Louis Armstrong New Orleans Airport and saw no passengers waiting at his gate.
While he was at the gate, a Delta flight attendant asked him if he would like to go home the next day because he would be the only passenger on the flight.
According to WSB, other passengers were scheduled to be on Flight 2426 but because of the flight being delayed a couple of times, they decided to take an earlier flight or postpone until the next day.
Schneider opted to keep his original reservation and had the entire flight to himself.
“At first I felt really bad because I didn’t want the plane to only make the trip because of me,” he told WSB. “But after talking to a flight attendant at the gate, she said the plane needed to go back to Atlanta anyway. She called this an empty leg flight.”
Schneider said the flight attendant told him she had never seen something like this in 17 years.

A Delta Airlines flight travelling from New York City to Reykjavik, Iceland, made an unscheduled stop in Happy Valley-Goose Bay on Wednesday.
Flight 446, carrying 183 passengers, stopped at Canadian Forces Base Goose Bay for more than eight hours Wednesday after landing at the base at about 4 a.m. ET due to deteriorating weather conditions in Iceland.
The plane eventually left the runway at about 1:30 p.m. ET.

A Delta flight from San Antonio to Los Angeles, diverted the plane to Tucson International Airport due to verbal threat from a passenger.
The suspect appeared to have a breakdown and was uncooperative. A flight attendant reported the suspect made verbal threat to the aircraft. First contact about this incident was about 4:30 p.m. said officials and was due to interference with the flight crew.

A Bombardier Challenger business jet aircraft operated by ExecuJet and chartered by Basil Read departs St Helena, having arrived at 11.38hrs yesterday (Monday 20 June 2016) to renew and relieve Airport staff.
On landing at St Helena Airport, the crew reported an uneventful landing operation with nothing abnormal experienced.
EMERGENCY Vietnam Airlines #VN1201 made an emergency landing because of cracked cockpit glass
A plane from Hanoi to Can Tho on June 21 morning had to land in HCM City as the cockpit glass was cracked.
At 7:30am, flight VN1201 took off from Noi Bai International Airport (Hanoi) to Can Tho, with 177 passengers and 7 crew members on board. The aircraft’s registration number was VNA-351.
The Airbus 321 from Hanoi to Can Tho City was cruising at an altitude of over 30,000ft when the cockpit glass cracked. The crew decided to land the plane at the international airport of Tan Son Nhat.
All passengers were transferred to another aircraft to fly to Can Tho at 11.25am.
The cause of the incident has yet to be identified. The aircraft was towed to the repair workshop.

Emirates has released a 360-video tour of the cockpit of the world’s biggest passenger plane – the Airbus A380 – featuring Kiwi pilot, Thomas Ziarno. Captain Ziarno and first officer Abdulrahman Mohamed Al Busaeedi from the United Arab Emirates explain the the controls of the super jumbo. Emirates is the biggest operator of A380s in the world and flies them to Auckland via Australia from Dubai and later this year will fly directly from the UAE. NEWS Brawl breaks out between passengers on Hainan Airlines flight #HU7737 in China
Police had to be called and five people were escorted off an airplane after a fight broke out in China on Saturday.
The brawl reportedly happened because a woman kept complaining about the delayed flight from Beijing to Xi’an.
When another passenger on the Hainan Airlines flight became involved she started hitting him with her bag, the Mail Online reported.
Flight HU7737 was due to take off at 9:50pm on Saturday but boarding was delayed until 12:30am on Sunday.
NEWS: KLM to launch new daily Dublin to Amsterdam service
KLM is set to launch a new service which will take off from Dublin Airport for the first time in half a century after sealing a deal on a new route to Amsterdam.
The Dutch flag carrier will start its twice daily service between the Irish capital and Schiphol Airport on October 30.
The flights will link into KLM’s long haul routes, opening up “easy connections” to destinations such as Johannesburg, Havana, Beijing and Hong Kong, the airline says.
Return fares between Dublin and Amsterdam will be upwards of €99.

BREAKING A passenger died on board Emirates #EK21 which just landed at Manchester
A passenger died on board Emirates flight EK21 from Dubai to Manchester. The reasons of the death are still not clear but the passenger was already ill when boarding at Dubai International Airport (DXB) and was traveling with a friend.
The Boeing 777-300 ER (reg. A6-EGT) landed at 7.30. All passengers disembarked.
UPDATE 07:47 UTC The body is about to be removed from the aircraft.

NEWS Iran to Buy 100 Boeing Airliners
Iran has reached a deal to buy 100 planes from U.S. plane maker Boeing, and the two sides are awaiting approval by U.S. Treasury authorities, the head of Iran’s Civil Aviation Organisation said in remarks published by state media on Sunday.
So far, Boeing has only been granted permission to present its products to Iran air and a handful of other airlines as it tries to catch up with Europe’s Airbus, which won a provisional deal earlier this year for 118 jets worth $27 billion.
Reuters reported on June 6 that Iran was edging towards a historic agreement to buy jetliners from Boeing for the first time since the 1979 Islamic Revolution and that a deal for more than 100 aircraft could be reached fairly soon.
“Of the 250 (passenger) planes in Iran, 230 have to be replaced” Ali Abedzadeh, the head of Iran’s Civil Aviation Organisation, told the state-run daily newspaper Iran, adding that a written agreement had been signed with Boeing to buy 100 aircraft.

NEWS Travel chaos for British Airways passengers after an IT glitch causes long queues at check in desks
Worldwide IT glitch at British Airways check-in desks has been causing delays at Gatwick and Heathrow airports this weekend.
The airline says the glitch has now been fixed – but delays are still ongoing.
According to the messages the airline posted online, the system is now back up and running but there are still delays due to the backlog caused by the glitch.
The airline has been apologising to the frustrated passengers on Twitter.
A spokesperson for British Airways said: ‘We have now resolved the IT issue which affected some of our check-in and boarding gate systems at a small number of airports.’
‘We are sorry for the disruption to our customers’ travel plans and our teams are working very hard to get customers checked-in as quickly as possible.’

Gothenburg Landvetter receives 3 new gates
To accomodate the greater amount of passengers flowing through Gothenburg Landvetter airport every day, month and year plans are now ready for an expansion of the airport.
An investment nearing 1.9 billion Swedish Crowns is needed to put the plans into reality.
The airport has grown steadily since the last 20 years with a bigger growth noticed in the later years than ever. The 40-year old terminal has a few points which has reached their limit.
The next step will be an expansion of the terminal facing south. It will include three new gates which will be able to accomodate bigger aircaft as well as more passengers. The project is supposed to be finished at year 2020.

Thomson Airways flight has been forced to divert to Gatwick after being hit by lightning.
The 737 was flying from Manchester to Morocco when it was forced to make an emergency landing after being hit by a bolt of lightning at 6:42 last night.
It had been an hour into the flight when they made the unexpected landing.

A routine flight bound for Los Angeles was grounded for 8 hours after the engines sustained damage.

The flight was originally scheduled to depart at 9:15am on Tuesday morning but that time was revised to 4:00pm.
According to Qantas, the flight departed at 5:23pm (local time) and arrived in Los Angeles at 2:25pm.

Ryanair has called again on the European Commission to take action to prevent the skies over Europe being closed once more by French air traffic controllers’ union strikes, targeting the European Championships. Ryanair has been forced to cancel 44 flights on Tuesday, to/from and over France, in what will be the 50th French strike since 2009, and tenth in the past 12 weeks.
EMERGENCY China Southern Airlines flight #CZ8367 made an emergency landing after passenger tried to force way into cockpit
A commercial flight made an unscheduled landing after a passenger tried to force his way into the cockpit, telling fellow travellers he wanted to fly the plane .
The incident happened just minutes after take-off, with dramatic footage from the aeroplane cabin showing the moment the apparently disturbed young man was subdued by other passengers and flight crew.
Reports said the scare occurred on China Southern Airlines Flight CZ 8367 which left the city of Wuhan, capital of Central China’s Hubei Province, bound for Harbin, capital of China’s north-eastern Heilongjiang Province.
The captain of the China Southern Airlines flight decided an unscheduled landing, which took place 20 minutes later in the city of Zhengzhou, capital of neighbouring Henan Province in Central China.
#OnThisDay in 1996, Garuda Flight 865 overruns the runway at Fukuoka, Japan
20 years ago today, Garuda Indonesia Airways Flight 865 (PK-GIE) was a scheduled international flight from Fukuoka, Japan to Jakarta, Indonesia with a stopover in Bali, Indonesia. The McDonnell Douglas DC-10-30 was carrying 260 passengers & 15 crew members at the time of the accident.

Garuda Flight 865 pushed back from gate 5 at Fukuoka International Airport at 11:55 local time. The crew reported “Ready for taxi”. Fukuoka ground ATC instructed the aircraft to taxi via E2 and to contact tower. The crew taxied to runway 16 and were instructed by the tower controller to hold short for another plane to land. The crew taxied into position and held.
At 12:06:53 take off clearance was given. The DC-10 accelerated for takeoff. The nose was raised and at a speed of 158kts the first officer called “Rotate” at 12:07:40.
Three seconds later, at a radio altitude of 9 feet, a fan blade on the 1st stage HP turbine from the no.3 engine separated. The N1 dropped to 23.7% within a few seconds.
At 12:07:45 the flight engineer called “Engine failure number one”. The crew attempted to abort take off at speeds nearing V2 (the speed you can safely climb with one engine inoperative). The aircraft contacted the runway one second later at a vertical acceleration force of 2.1 Gs.
Attempts were made to stop the aircraft on the runway using the thrust reversers, brakes and spoilers. But the crew were unable to stop the aircraft within the boundaries of the runway.
The DC-10 skidded off the runway through a ditch, fence and a road, before coming to a halt 620 m (2,030 ft) past the runway threshold. Damage done to the aircraft during the slide across the ground caused the landing gear to break off and both wing-mounted engines to be torn from the wings.
The fuselage broke into two places, at about the wing root trailing edge, and at approximately 10.4 metres (34 ft) aft of the wing root trailing edge. The resultant fire destroyed the areas between the hull fractures, and other areas of the aircraft. Three passengers died as a result.
AAIC Investigators revealed that the turbine blade that failed, had operated for 30913 and 6182 cycles. General Electric advised customers to discard blades after about 6000 cycles.
The final report concluded that pilot error and the failure of the maintenance and flight operation sections of the airline in properly coordinating matters resulted in the accident.

NEWS SAS Swedish Pilots Strike, flights and passengers affected
Scandinavian airlines says its flights operated by swedish pilots are being canceled due to a strike. Nearly 160 flights and 20000 passengers are affected this Saturday.
The Stockholm based subsidiary says that routes with pilots from other nationalities than Sweden are not affected.
The Swedish Pilots’ Union demand a 3.5 percent increase in salary while SAS only offers a 2.2 percent increase. This according to the Swedish news agency TT.

Arrived on Friday afternoon, the plane of the Wales football team has experienced a difficult landing in Bordeaux.
First report shows that after landing, the pilot committed an error at the end of the runway. One of the wheels mired.
The players still able to get out were finally able to get off the plane 30 minutes later to board the bus to take them to their destination.
The team will meet Slovakia this Saturday in Bordeaux stadium.
NEWS Russia’s Su-27 Fleet Grounded Over Deadly Crash
Head of the Russian Air Force, Colonel-General Viktor Bondarev has ordered the grounding of the country’s entire Su-27 fleet, until the reasons behind the crash that led to a pilot’s death on Thursday are clear, the TASS news agency reported. The Defense Ministry confirmed that the jet crashed while returning to the airbase after a planned flight. Six jets from the Russian Knights squad took part in the opening ceremony of a monument dedicated to Russian aviators in the village of Anushkino earlier on Thursday. Only five of them returned to the airbase, TASS reported.
ALERT #MH370 search: New debris found on Madagascar beach
New pieces of debris have been found in Madagascar by a man searching for parts of missing flight MH370. Blaine Gibson, who has already found possible debris in Mozambique, made the latest discovery on the east coast of Madagascar. Boeing 777 flying from Kuala Lumpur to Beijing, had 239 people on board when it vanished in March 2014.
NEWS Jet Time to reduce fleet size
The danish company jet time which for the most part flies charter flights for a few Norwegian carriers as well as subcontractor for SAS has announced that it will be reducing its fleet by 30%.
This is the next part of their work to restore its economy. Last year jet time lost roughly 16 million dollars.
A clear announcement has been made by the company: to reduce its fleet of 27 aircraft down to 18 aircraft. It’s the old Boeing 737-300s that are supposed to be retired, as well as three of the ATR 72-500s.

EMERGENCY Ryanair flight made emergency landing in Cologne due to smoke in cabin
Ryanair flight from to Porto made an emergency landing Tuesday at about 17:00 in Cologne (Germany).
The plane emergency landed after smoke was detected. Portuguese passengers told about a “burning smell” after takeoff.
BREAKING A ramp agent was struck by lightning at Frankfurt airport
Yesterday, a storm paralyzed the airport during one hour. Several flights were canceled, others delayed.
At 21:47, a ramp agent was struck by lightning on the tarmac of Frankfurt Airport. The man he was rolling away a Boeing from the gate B45.
The lighting struck his headset connected to the pilots when he was standing by the plane. The a 44 year old man was seriously injured and immediately sent to the hospital. He was still able to talk to the doctor.
ALERT Chinese jet made intercept of US Air Force plane
A U.S. Air Force RC-135 reconnaissance aircraft flying Tuesday in international airspace over the East China Sea was intercepted in an “unsafe manner” by a Chinese J-10 fighter jet, several defense officials tell CNN.
The Chinese jet was never closer than 100 feet to the U.S. aircraft, but it flew with a “high rate of speed as it closed in” on the U.S. aircraft, one official said. Because of that high speed, and the fact it was flying at the same altitude as the U.S. plane, the intercept is defined as unsafe.
The officials did not know if the U.S. plane took any evasive action to avoid the Chinese aircraft or at what point the J-10 broke away. It is also not yet clear if the U.S. will diplomatically protest the incident.
Officials said the RC-135 was on a routine mission.

BREAKING F-16 fighter jets collide in mid-air over Georgia
Two F-16 fighters collided and crashed in Georgia late Tuesday, officials said.
The South Carolina Air National Guard said the jets were performing “routine night-flying operations” when the incident occurred at around 9:15 p.m. ET over a military operating area in Jefferson County, Georgia.
“The pilots of each of the single-seat aircraft ejected safely,” it said in a statement.
The U.S. Air Force will conduct a detailed safety investigation.
BREAKING Bomb threat forces EgyptAir flight to make emergency landing in Uzbekistan
Reuters is reporting an EgyptAir flight to China was forced to make an emergency landing in Uzbekistan few hours ago after the airline received a call claiming that there was a bomb on board, two Egyptian aviation officials said.
The plane carrying 118 passengers had been evacuated and searched. No explosives were found.

NEWS American Airlines planes will get ViaSat’s powerful WiFi
American Airlines is equipping a fleet of Boeing 737 MAX planes with in-flight WiFi other than Gogo’s. It has inked a deal with California-based company ViaSat, which also provides internet connection to some of Virgin America’s newest A320 planes. ViaSat promises an “at home” experience by providing you with a (12 Mbps) connection strong enough to stream movies and music. It uses a high-capacity satellite system, and it’s launching another satellite in 2019 to make its connection even faster. You can’t confirm that speed on an AA plane just yet, though: the 100 737 MAX planes the carrier ordered won’t be delivered until September 2017.

All passengers on an Air Canada flight from Paris to Toronto were forced to be rescreened on the tarmac after arriving in Canada last Friday after more than a dozen flyers did not receive proper security screening in France before getting on board.
The airline said crew aboard Air Canada Flight 881 on June 3 learned mid-flight that 17 of its passengers did not go through the required preboard security screening at Paris’s Charles de Gaulle airport. There were 325 passengers and 12 crew on board the flight.
Air Canada said the passengers were “misdirected” by staff at Charles de Gaulle after getting off a connecting flight from Sri Lanka and were supposed to go through more security. But that additional screening never happened.
Peter Fitzpatrick, a spokesman for Air Canada, said those passengers had been screened prior to their flight from Sri Lanka and did not leave the “secure zone” at Charles de Gaulle. He said their bags were properly screened in Paris.

EMERGENCY Fire forces passengers off American Airlines flight at Sea-Tac airport
All passengers had to deplane after a fire broke out outside an American Airlines flight at Sea-Tac Airport Monday morning.
Sea-Tac Airport spokesman Perry Cooper said a ground start unit that is used to help start the engines caught fire at about 8:30 a.m. The starter hooks up to the aircraft externally, and when it caught fire, it was pulled away from the plane.
Port of Seattle fire responded and doused the flames. Everyone deplaned safely.
There was no damage to the aircraft.

VIDEO Airbus A300 is sunk off Turkey to become artificial reef
Turkish authorities have sunk an A300 Airbus jet off the Aegean coast with the aim of attracting more diving tourists to the region.
The plane was put to the bottom of the sea off the resort of Kuşadası, in Aydin province, 50 miles south of Izmir.
The sinking of the plane, which is 54 metres (177ft) long and has a wingspan of 44 metres, is aimed at promoting artificial reef diving, which is hugely popular with experienced divers. Once sunk, the plane acts like a reef, becoming a magnet for underwater flora and fauna.
The 36-year-old aircraft was bought by Aydin municipality from a private aviation company for 270,000 Turkish lira (£64,000).
Three small planes have been sunk off Turkish resorts of recent years in the hope of promoting dive tourism but this is the first time Turkey has sent such a large aircraft to the bottom of the sea.

ALERT Extreme turbulence on board Avianca #AV965 from Lima to Buenos Aires injuring 20 passengers
Avianca #AV965 was crossing the Andes and going to Santiagowhen when Airbus A330 hit extreme turbulence.
The incident left at least 20 minor injuries and 8 crew members suffered injuries because flight attendants were serving the food on board.
As reported by the airline, the event occurred when the aircraft was at cruising over “Laguna Mar Chiquita” northeast of Cordoba, Argentina.
In a statement, Avianca told the Airbus A330 was subjected to a review of fuselage structure.
#OnThisDay in 1971, Hughes Airwest Flight 706 collides with an U.S F-4 Phantom II
45 years ago today, Hughes Airwest Flight 706 (N9345) was a scheduled flight from Los Angeles, California to Seattle, Washington with several intermediate stops. The McDonnell Douglas DC-9-31 was carrying 44 passengers & 5 crew members at the time of the accident.

The aircraft was piloted by Captain Theodore Nicolay, 50 who had logged 15,490 hours of total flying time, with 2,562 hours in DC-9s. His co-pilot was First Officer Price Bruner, 49, who had 17,128 total hours flying and 272 hours in DC-9s.
Hughes Airwest Flight 706 departed from Los Angeles at 6:02pm (PDT) for Salt Lake City, Utah, the first of five intermediate stops en route to Seattle. Control of the flight was transferred to Los Angeles Air Route Traffic Control Center at 6:06pm. Flight 706 passed through 12,000ft at 6:09pm. The ATC instructed to head 040 (magnetic) until receiving the Daggett VOR, then direct. Flight 706 acknowledged.
The U.S. Marine Corps F-4B-18-MC Phantom ll, Bureau Number 151458, coded ‘458’. The aircraft was piloted by 1st Lt. James R. Phillips, 27 and 1st Lt. Christopher E. Schiess, 24. The jet and its crew were based at MCAS El Toro.
‘458’ was part of a cross-country flight of two aircraft when its radio failed while landing at Mountain Home Air Force Base in southwest Idaho. The aircraft was ordered to effect repairs at Mountain Home AFB and then return to MCAS El Toro. Diagnostic tests at Mountain Home revealed that the aircraft had an inoperative radio, inoperative transponder, oxygen system leak and a degraded radar system. Maintenance personnel were able to fix the radio and confirm the oxygen leak, but the base did not have the necessary personnel to repair either the transponder or the radar.
Lt Phillips received permission from his superiors to fly the F-4B with an inoperative transponder as fighter proceeded to NAS Fallon in Nevada, the oxygen leak worsened until the system was disabled completely. The pilot was instructed to fly at low altitude. The Phantom ll departed NAS Fallon at 5:16pm following a flight plan routing across the Fresno, Bakersfield and Los Angeles air corridors.
Near the Bakersfield Flight Service Station, the crew of ‘458’ decided to deviate east from their flight plan to avoid heavy air traffic in the Los Angeles area. Lt. Phillips was forced to climb to 15,500ft from 1,000ft because of deteriorating weather conditions. Meanwhile, shortly after takeoff, Flight 706 received two radar traffic advisories, neither of which indicated the presence of ‘458’.
Soon after reaching 15,500ft, the fighter’s DME (radio) showed MCAS El Toro was 50 miles (80km) away. The pilot of ‘458’ then performed an aileron roll in order to allow the pilot to observe any air traffic above or below the aircraft. Lt Schiess, the radar intercept officer was operating the fighter’s radar, which was unable to detect any aircraft due to its deteriorated condition. Because of the stowed position of the scope, he had been leaning forward and looking downward at the instrument. Between three and ten seconds prior to the collision, he glanced up and observed the DC-9 in his peripheral vision and shouted a warning to the pilot. The pilot attempted an evasive roll but was unable to clear the oncoming airliner.
At 6:11pm, Flight 706 and ‘458’ collided at about 15,150ft over the San Gabriel Mountains in the vicinity of Duarte. The collision tore the F-4 tail off, and the DC-9’s cockpit was shorn off as a result. Flight 706 cartwheeled through the air and plunged downwards. The aircraft crashed onto Mount Bliss in the San Gabriel Mountains killing everyone on board.
On the F-4B, Lt Schiess, the Radar Intercept Officer ejected and parachuted to safety. Lt Phillips, the pilot was unable to eject in time and was killed. The F-4B also crashed on Mount Bliss, approximately 1 mile from the airliner wreckage.
The National Transportation Safety Board determines that the probable cause of this accident was the failure of both crews to see and avoid each other, but recognizes that they had only marginal capability to detect, assess and avoid the collision. Other causal factors include a very high closure rate, commingling of IFR and VFR traffic in an area where the limitation of the ATC system precludes effective separation of such traffic, and the failure of the crew of BuNo458 to request radar advisory service, particularly considering the fact they had an inoperable transponder.

EMERGENCY Malaysia Airlines A380 #MH1 from London hits severe turbulence, injuring passengers and crew
A Malaysia Airlines A380 operating flight MH1 from London-Heathrow to Kuala Lumpur encountered severe turbulence over the Bay of Bengal on Sunday.
The superjumbo aircraft was carrying 378 passengers and crew onboard.
The airline reported the seatbelt sign was on at the time of the incident. According to Malaysian newspaper The Star, around 34 people were injured.
Malaysia Airlines confirmed a “small number of affected passengers and crew have been treated by medical officers.”
NEWS Air France pilot unions call for strike during Euro 2016
Air France’s pilot unions have called on members to strike during the first week of the Euro 2016 soccer championship in a protest over pay, although the airline’s chief executive said fans could just drive or take the train. The unions said in a joint statement they planned to strike from June 11 until June 14 and might schedule further strikes a few days later if necessary. The soccer tournament begins in Paris on June 10 and will run until July 10.
