#OnThisDay in 1996, Ethiopian Airlines Flight 961 ditches in the Indian Ocean
20 years ago today, Ethiopian Airlines Flight 961 (ET-AIZ) was a scheduled passenger flight from Bole International Airport, Addis Ababa, Ethiopia to Port Bouet Airport, Abidjan, Côte d’Ivoire with stopovers in Nairobi, Kenya; Brazzaville, Republic of the Congo & Lagos, Nigeria. The Boeing 767-260ER was carrying 163 passengers & 12 crew members.
ET-AIZ, the aircraft involved in the hijacking.
HIJACKING
The cockpit crew of Ethiopian Airlines Flight 961 consisted of Captain Leul Abate, 42 an experienced pilot with over 11,500 total flight hours, was the pilot-in-command. The First Officer was Yonas Mekuria, 34. He had flown more than 6,500 hours.
Ethiopian Airlines Flight 961 took off at 08:09 UTC at Bole International Airport, Addis Ababa, Ethiopia. The flight had been delayed in order to allow a connecting flight to feed passengers. Twenty minutes after takeoff, at about 08:29 UTC, one passenger stood up from his seat and ran up the aisle to the cockpit, and the two other passengers followed him heading for the cockpit. While rushing to the cockpit one of the men said “Everyone should be seated, I have a bomb!”.
Then they opened the cockpit door and stormed in. They declared to the pilots that there were eleven hijackers on board and beat the First Officer and forced him out of the cockpit. They then grabbed the fire axe and fire extinguisher bottle from their respective stowages and ordered the pilot-in-command (Captain Leul Abate) to change direction and fly to Australia. The Abate explained to the hijackers that he had not enough fuel to reach Australia and demanded to make a refueling stop at Mombassa. The hijackers refused and continued arguing with Abate. They insisted that they had learned from the in-flight magazine that a Boeing 767 could fly 11 hours without refueling.
After passing Dar es Salaam one of the hijackers ordered him to fly away from the coast, head to Australia and indicating to the altimeter not to descend below FL390. Abate turned left towards the Comoros Island. The lead hijacker was sitting in the first officer’s seat and was fiddling with the aircraft’s controls, kicking the rudder, whilst also drinking whisky. Abate kept on telling them that he was running short of fuel pointing to the fuel quantity indicators, but the hijackers did not listen. The leader continued fiddling with the controls, trying to turn the aileron and pulling the reverse thrust lever at random.
As the flight came over the Comoros Islands the Abate saw the Moroni International Airport runway and circled 15-20 nm south of the field. Then the LOW FUEL CAUTION came on. Abate pleaded to land because of low fuel. The hijackers were unconcerned and only insisted that the pilot not descend below FL390. At about 11:41 UTC the right engine ran down to wind milling speed. Abate showed the red warning message for the right engine on the EICAS to the hijacker.
At this moment, the hijacker left the right seat and went to the cabin door to discuss with the other two hijackers. This gave the captain the opportunity to pick up his microphone and address the passengers: “….ladies and gentlemen this is your pilot, we have run out of fuel and we are losing one engine this time, and we are expecting crash landing and that is all I have to say. we have lost already one engine, and I ask all passengers to react ….. to the hijackers ….” The hijacker then came back to the cockpit and hit the microphone out of the pilot’s hand. After the right engine failed, the pilot started to descend the aircraft in order to increase speed, but the hijacker again interfered and violently played with the controls which resulted in improper control inputs. As a result the autopilot was disconnected and the flight became erratic with the airspeed varying between 216 and 336 kts.
As the pilot regained control of the aircraft, the left engine went dead. The hijacker kept on instructing the pilot not to descend and again went to the cabin. Upon returning to the cockpit he saw that the altitude was decreasing, and angrily shouted at the pilot not to go any lower. The pilot said that the fuel was already finished and that the engines were without power. This time the hijacker instructed the captain not to touch the controls, and threatened to kill him. The captain said, “I am already dead because I am flying an airplane without engines power.”
The first officer, who had earlier been forced out to the First Class cabin, got up and, via the right aisle, went to the rear of the aircraft where he saw that a lot of economy class passengers had their life jackets on and that some had already inflated them. The first officer, along with the cabin crew members, helped the passengers to deflate the life jackets and showed them how the jackets should be re-inflated and how to assume the brace position during impact. While returning to the front of the aircraft, they repeated the same instructions as many times as they could.
About less than 2 minutes before the ditching, the co-pilot forced his way to the cockpit shouting “let me help the pilot …”. After adjusting his seat and seat belts the pilot asked him for help since the controls were heavy. The hijackers still kept on struggling with the controls. By now, the aircraft was descending into the Indian Ocean over the Comoros Islands. The aircraft now had only standby instruments and RAT (Ram Air Turbine). The altimeter was indicating 150 feet and the airspeed was 200 kts. By this time the flight crew had been left alone to assume control. They turned the aircraft to the left in order to parallel the waves. However, the aircraft brushed the water in a left-wing-low attitude. It was then held straight and level after which it broke into four sections and came to rest in the sheltered waters 500 m off Le Galawa Beach.
Of the 175 occupants 6 crew members and 119 passengers were fatally injured in the accident. Six crew members and 38 passengers sustained serious injuries, 2 passengers sustained minor injuries and 4 passengers received no injury.

NEWS Transasia airline shuts after crashes
Taiwanese airline Transasia is shutting down after two fatal crashes and heavy losses. The carrier, which flew to cities in China, Japan and south east Asia, said it had not been able to raise more money or turn around its finances.
Transasia, Taiwan’s third largest airline, had already cancelled all flights on Monday.
Earlier this year, regulators ordered it to review safety protocols and pilot training to cut “imminent risks”.
Those recommendations, from the country’s Aviation Safety Council, followed an investigation into an incident in July 2014, when Transasia flight 222 crashed in a heavy storm near Magong Airport on Taiwan’s Penghu island, leaving 48 dead. Ten survived.
Tragedy struck again in February 2015, when TransAsia flight 235 clipped a bridge in Taipei and came down in the Keelung River, killing 43 people. That incident was captured on the dashboard camera of a passing vehicle.
Closing the airline had been a “very painful choice”, chief executive Daniel Liu said.
Passengers with tickets for future flights would be refunded, the airline said. The future of its 1,700 staff members was unclear.

NEWS Emirates moves to an all Airbus A380 and Boeing 777 fleet
In line with its vision to offer a superior passenger experience and improve environmental performance through a modern and state of the art wide-body aircraft fleet, Emirates has retired the last Airbus A330 and A340 aircraft in its fleet from active service. This makes Emirates the first and only airline in the world to operate a fleet of all Airbus A380 and Boeing 777 aircraft for its passenger flights.
Emirates recently retired A6-EAK from operational service. The aircraft was the last of the 29 Airbus A330 aircraft that had been operating as part of its fleet. A6-EAK joined Emirates in 2002 and had flown for over 60,000 hours travelling close to 45 million kilometres in 14.5 years. That distance is equivalent to almost 60 return journeys between the Earth and the Moon. Emirates has also phased out A6-ERN, the last serving Airbus A340 in the fleet which had joined the airline in 2004, originally manufactured in 1999.
Since January 2015, Emirates has retired 18 A330 and 5 A340 aircraft from its fleet. The average age of the Airbus A330 and A340 aircraft phased out from the fleet is 16.5 years- a figure which is well below the industry standard retirement age of 25 years. In addition to the aircraft that have been retired from active service since January 2015, Emirates plans to further phase out some 25 aircraft over the course of 2017 and 2018 to ensure that the operating fleet remains modern and efficient while offering customers a higher level of comfort and safety.
The retirement of older aircraft is balanced by the induction of younger, more modern aircraft into the fleet. This has resulted in Emirates operating one of the youngest fleets in the industry with an average age of 5.2 years. The two youngest aircraft in the fleet- Emirates’ 85th A380- the first of the new generation A380 aircraft delivered in October 2016 and Emirates’ 125th Boeing 777-300ER- are less than 2 weeks old. Emirates’ fleet of all Airbus A380 and Boeing 777 aircraft will also have a smaller environmental impact as both aircraft types have better fuel efficiency and emissions performance than the retired aircraft.
For the calendar year 2016 Emirates will have taken delivery of 36 new aircraft – 20 Airbus A380s and 16 Boeing 777. This also includes the next generation Boeing 777-300ER aircraft – with upgraded business class seats and other features including a lower fuel burn ratio – to be delivered from November 2016.
Emirates is currently the largest operator of the Airbus A380 and Boeing 777 aircraft with 85 Airbus A380s and 160 Boeing 777s in its fleet. Out of the 234 aircraft worth over USD 112 billion that Emirates has in its order book, 150 will be the new Boeing 777X aircraft that will be delivered starting 2020. The aircraft will feature a range of passenger focused amenities on board including larger windows, higher ceiling, and a wider cabin in addition to being more fuel efficient than older aircraft.

#OnThisDay in 2001, American Airlines Flight 587 crashes in Queens, New York
15 years ago today, American Airlines Flight 587 (N14053) was a scheduled passenger flight from John F. Kennedy International Airport, New York City to Las Américas International Airport, Dominican Republic. The Airbus A300B4-605R was carrying 251 passengers & 9 crew members.
N14053, the aircraft involved in the accident.
ACCIDENT
First Officer Sten Molin with his niece Casey Parsons in 2000.
American Airlines Flight 587 consisted of two flight crew members Captain Ed States, 42 and First Officer Sten Molin, 34. The First Officer was the pilot flying.
Flight 587 was scheduled to leave John F Kennedy at 08:00 for a flight to Santo Domingo, Dominican Republic. The boarding process at gate 22 took a little longer than planned due to additional security procedures that delayed boarding. The gate was closed at 08:38 and pushback from the gate was accomplished at 9:00.
American Airlines Flight 587 taxiing to Runway 31L at 8:59AM (Timestamp not adjusted for Standard Time)
The crew taxied to runway 31L behind Japan Airlines Flight 047, a Boeing 747-400 bound for Tokyo, Narita. JL047 was cleared for takeoff at 9:11am, as the 747 climbed, the tower contacted Flight 587’s pilots and warned them about potential wake turbulence from the 747.
At 9:13:28 Flight 587 was cleared for takeoff from Runway 31L. The aircraft left the runway at 9:14:29, about 1 minute and 40 seconds after the JAL flight. From takeoff, the plane climbed to an altitude of 500 ft above mean sea level (msl) and then entered a climbing left turn to heading of 220. At 9:15:00, the pilot made initial contact with the departure controller informing him that the airplane was at 1,300 ft and climbing to 5,000 ft. The departure controller instructed the aircraft to maintain 13,000 ft.
At 9:15:36, when the aircraft hit wake turbulence from the JAL flight just in front of it. The first officer attempted to stabilise the aircraft with alternating aggressive rudder inputs from left to right. This continued for at least 20 seconds, until 9:15:56, when the stress of the first officer’s repeated rudder movements caused the lugs that attached the vertical stabilizer and rudder to fail. The stabilizer separated from the aircraft and fell into Jamaica Bay, about one mile north of the main wreckage site.
American Airlines Flight 587 ATC Recordings
Eight seconds later, the stall warning sounded on the cockpit voice recorder. At the moment the stabilizer separated from the aircraft, the plane pitched downwards, headed straight for Belle Harbor. As the pilots struggled to control the aircraft, it went into a flat spin. The resulting aerodynamic loads sheared both engines from the aircraft seconds before impact. The engines landed several blocks north and east of the main wreckage site. Losing the engines cut off power to the FDR at 9:16:00, while the CVR cut off at 9:16:15 moments before impact.
The fuselage slammed into Belle Harbor on Beach 131st Street, instantly destroying three houses. All 260 people aboard the plane and five people on the ground were killed instantly, and the impact forces and post-crash fire destroyed the wreckage.
INVESTIGATION
The investigation into the crash of American Airlines Flight 587 was led by the National Transportation Safety Board (NTSB).
They concluded that the probable cause of the in-flight separation of the vertical stabilizer was the result of the loads beyond ultimate design that were created by the first officer’s unnecessary and excessive rudder pedal inputs. Contributing to these rudder pedal inputs were characteristics of the A300-600 rudder system design and elements of the American Airlines Aircraft Manoeuvring Program.

#OnThisDay in 1996, Saudi Flight 763 & Kazakhstan Flight 1907 collides over Charkhi Dadri, India
20 years ago today, Saudi Arabian Airlines Flight 763 (HZ-AIH) was a scheduled international passenger flight from Indira Gandhi International Airport, Delhi, India to Dhahran International Airport, Saudi Arabia. The Boeing 747-168B was carrying 289 passengers & 23 crew members.
HZ-AIH, the accident aircraft involved seen at London Heathrow Airport in March 1986.
ACCIDENT
Saudi Arabian Airlines Flight 763 departed Delhi at 18:32 local time. The cockpit crew consisted of Captain Khalid Al Shubaily. First Officer Nazir Khan and Flight Engineer Edris. The captain was a veteran pilot with more than 9,800 hours to his credit. 215 Indians that boarded Flight 763 worked in Saudi Arabia; many of them worked or planned to work in blue collar jobs as house maids, drivers and cooks.
At the same time Kazakhstan Airlines Flight 1907 (UN-76435) from Shymkent International Airport, Kazakhstan was descending into Indira Gandhi International Airport. The Ilyushin II-76TD was carrying 27 passengers & 10 crew members. The captain was also highly experienced with more than 9,200 flight hours.
UN-76435, the aircraft involved seen in 1994.
Kazakhstan 1907 was cleared to descend to 15,000 ft when it was 74 nautical miles from the beacon at Indira Gandhi. While Saudi 763 was travelling on the same airway as Kazakhstan 1907 but in the opposite direction, was cleared to climb to 14,000 ft.
About 8 minutes later, around 18:40 Kazakhstan 1907 reported having reached its assigned altitude of 15,000 ft but it was actually lower at 14,500 ft and still descending. At the same time, ATC controller VK Dutta advised the flight, “Identified traffic 12 o’clock, reciprocal Saudi Boeing 747, 10 nautical miles. Report in sight.”
When the controller called Kazakhstan 1907 again, he received no reply. He warned of the other flight’s distance, but it was too late. The two aircraft had collided, the tail of Kazakhstan 1907 cut through Saudi 763 left wing and horizontal stabiliser. The crippled Boeing 747 quickly lost control and went into a rapidly descending spiral motion towards the ground with fire trailing from the wing. The Boeing 747 broke up in the air under the stresses before the wreckage hit the ground at almost 705 mph. The Ilyushin remained structurally intact as it went in a steady but rapid and uncontrollable descent until it crashed in a field.
When the aircraft collided Captain Timothy J. Place, a pilot for the United States Air Force was making an initial approach in a Lockheed C-141B Starlifter when he saw that “a large cloud lit up with an orange glow”.
Rescuers discovered four critically injured passengers from the Ilyushin, but they all died soon afterwards. Two passengers from the Saudi flight survived the crash, still strapped to their seats, only to die of internal injuries soon after. All 312 passengers & crew on board Saudi 763 and all 37 passengers & crew on board Kazakhstan 1907 were killed. Making it the world’s deadliest mid-air collision.
INVESTIGATION
The investigation into the crash of the Charkhi Dadi mid-air collision was led by the Lahoti Commission, headed by then-Delhi High Court judge Ramesh Chandra Lahoti with help from UK Air Accident Investigation Branch (AAIB).
They concluded that the probable cause of the accident was the fault of the Kazakhstan 1907 commander who had descended from the assigned altitude of 15,000 ft to 14,500 ft and then subsequently 14,000 ft and even lower. The report ascribed the cause of this serious breach in operating procedure to the lack of English language skills on the part of the Kazakhstan pilots; they were relying entirely on the radio operator for communication with the ATC.
Also, Indira Gandhi International Airport did not have secondary surveillance radar, which provides extra information such as the aircraft’s identity and altitude, by reading transponder signals; instead the airport had primary radar, which produces readings of distance and bearing, but not altitude. In addition, the civilian airspace around New Delhi had one corridor for departures and arrivals. Most areas separate departures and arrivals in separate corridors. The airspace had one civilian corridor because much of the airspace was taken by the Indian Air Force. Due to the crash, the air crash investigation report recommended changes to air-traffic procedures and infrastructure in New Delhi’s air-space.
- Separation of inbound and outbound aircraft through the creation of ‘air corridors’
- Installation of a secondary air-traffic control radar for aircraft altitude data.
- Mandatory collision avoidance equipment on commercial aircraft operating in Indian airspace.
- Reduction of the airspace over New Delhi that was formerly under exclusive control of the Indian Air Force.
The civil aviation authorities in India made it mandatory for all aircraft flying in and out of India to be equipped with an airborne collision avoidance system.

ALERT Violent brawl on Ryanair flight forces emergency landing in Pisa
The footage was filmed by Tommy Engere, who described the brawlers as “gypsy-like. He said: “It started when an elderly woman moved seats and told me they’d hit her on the head then a huge fight broke out when two Belgian men tried to intervene. They were attacking everyone around them and one of the men even slapped a stewardess in the face.
The video shows the chaos on board as the large men punch each other in the enclosed space. Passengers can be heard screaming and urging the men to stop.
“They looked out of control as if they were on drink or drugs and the cabin crew did well to bring the situation under control,” Engere said. “They were eastern European – gypsy types – but spoke a bit of French as well, and were incredibly aggressive.”
The captain can be heard saying the fight was “not acceptable at all” before announcing the unscheduled landing.
