NEWS Hot air balloon catches fire and crashes in Texas, 16 people dead
At least 16 people were feared dead after a hot air balloon caught fire and crashed in a pasture near the central Texas city of Lockhart on Saturday morning in one of the deadliest balloon accidents on record.
The Federal Aviation Administration said the crash occurred near Lockhart, about 30 miles (50 km) south of Austin and at least 16 people were on board the balloon. It did not comment on fatalities.
“It does not appear at this time that there were any survivors of the crash,” Caldwell County Sheriff Daniel Law said in a statement.
Emergency responders in Texas said the fire hit the basket portion of the hot air balloon.

ACCIDENT
At 01:32 (05:32 UTC) Caribbean Airlines Flight 523 landed on runway 06 at Cheddi Jagan Airport (a 7,448ft long asphalt runway) but failed to stop on the runway surface. The aircraft overran the runway crashing through the perimeter fence and stopped 100 meters past the end of runway 06 after it went over a road and broke into two sections.
No deaths were reported. However, some passengers suffered broken legs. The majority of the injured were treated at Diamond Diagnostic Hospital then sent to Georgetown Public Hospital, where 35 passengers were treated for leg, back and neck injuries. The aircraft was damaged beyond repair.
INVESTIGATION
The Investigation was led by the Guyana Civil Aviation Authority with assistance from the National Transportation Safety Board (NTSB) and the Trinidad and Tobago Civil Aviation Authority (TTCAA).
It was concluded that the cause of the accident was the aircraft touching down far beyond the touchdown zone due to the captain maintaining excess power during the flare and not using the aircraft’s full deceleration capacity resulting in the aircraft over running the pavement and fracturing the fuselage.

NEWS First JetBlue flights to Cuba will launch next month beginning at $99 one-way
JetBlue’s first regularly scheduled flights to Cuba will begin next month, with fares beginning at $99 one-way.
JetBlue’s first non-charter Cuba route will launch Aug. 31, when it begins nonstop service between Fort Lauderdale and the central Cuban city of Santa Clara. JetBlue initially will operate three flights a week before going to daily service on Oct. 1.
That would appear to put JetBlue in line to be the first U.S. carrier to begin normal airline flights to Cuba since the Obama administration moved to normalize relations between the nations. American Airlines and Silver Airways also have announced their initial schedules for regular Cuba flights, though the first of those services don’t begin until a few days after JetBlue’s first flight.
JetBlue says service from Fort Lauderdale to two other Cuban cities will start in November. Daily flights to the interior city of Camagüey begin Nov. 3 while daily service to Holguín in eastern Cuba starts Nov. 10.

In the beginning of the week the Belarusian air carrier Belavia and Wargaming, the developer of the popular online game World of Tanks, announced a demonstration of a unique branded airliner Boeing 737-300.
The Boeing is painted in black, with the Belarusian ornament in orange on the fuselage and a white cornflower — one of the symbols of Belarus — on the tail. The inscription on the body of the plane says “The Game from Belarus Enjoyed Worldwide”.
It took 13 people, 300 litres of paint and 1800 man hours to create its unusual appearance.

ALERT Air Asia A330 comes within 152m of colliding with Jetstar A320 in Australia
A Jetstar and an AirAsia plane came dangerously close to a midair collision after taking off from the Gold Coast, China’s Xinhua news agency reported Australian authorities as saying.
It was reported that the AirAsia Airbus A330 (flight D7206) on its way to New Zealand came within 152 metres of a Jetstar flight that was about to land, causing alarms to sound in both planes’ cockpits.
The minimum vertical separation for major passenger aircraft in Australia is 305 metres. The incident occurred north of Coolangatta airport in Queensland.
Jetstar pilots were forced to take evasive action as the AirAsia plane encroached on its flight path, putting their aircraft into a climb to avert a possible catastrophe.

A Jetstar spokesman told NewsCorp that pilots of their A320, which can carry 180 passengers took corrective action to restore the safe distance between the two aircraft and landed on the Gold Coast without further incident.

In a statement, AirAsia X maintained that its pilots had followed instructions by the Collangatta airport Air Traffic Control, prior to a near midair collision with a Jetstar passenger aircraft in Queensland yesterday (21July16).
The pilots of flight D7 206 had followed the instructions laid out by the airport’s ATC. “AirAsia X pilots on board flight D7 206 received clearance for take off at Coolangatta airport on 21 July 2016 and followed the flight path according to the heading and altitude instructed by Air Traffic Control,” said the airline.
The company also confirmed that the pilots received warning from the traffic collision avoidance system. “The crew responded immediately to the resolution advisory by reducing the vertical speed according to procedures that they are trained for. “The aircraft continued its journey safely without any subsequent events,” said the statement.

NEWS Man angry at his flight being delayed beat up two police officers at Frankfurt Airport
The clip shows the man kicking and punching two officers who are incapable of subduing him at a Lufthansa check-in desk.
Other travelers in the background boo and heckle but fail to intervene.
The confrontation ends when the man suddenly drops to the floor. It is unclear whether he was tasered or whether he was just play-acting in order to portray himself as the victim.
The rest of the man’s family, including two women wearing hijabs then crowd around the police.
Some people claimed that the man was upset because security tried to check his wife’s veil, although there is no confirmation of this.
Frankfurt Police quickly responded to the video after it went viral in aFacebook post, urging people not to spread “anxiety” and “terror”.
“Several thousand passengers were stranded at the airport” due to “bad weather,” states the police response.

An American Airlines flight from Dallas-Fort Worth to Seattle declared an emergency and returned to DFW Airport after fire was seen shooting out of one of its engines.
The plane carried 130 passengers and a crew of six.
Passengers saw flames shooting from an engine of the Airbus 321 aircraft after they heard “a boom”.
American Airlines says the problem was caused by a compressor stall and that the engine was never on fire.

EMERGENCY Business plane skids off runway at Sugar Land airport (USA)
A plane skidded off the runway Tuesday afternoon while landing at an airport in Sugar Land, according to police.
According to Sugar Land police spokesman Doug Adolph, the twin-engine jet was attempting to land at the Sugar Land Regional Airport when it slid off the north of end of the runway and into a creek.
Adolph said two pilots and a passenger were on board at the time of the crash. Nobody was injured.
Federal Aviation Administration records showed that the tail number of the airplane was registered to Flex Jet in Cleveland, Ohio. Officials said that is where the flight originated.
NEWS London City Airport Granted £344m Expansion
Plans for a £344m expansion of London’s City Airport have been given clearance by the Government.
The decision, the Treasury said, would see the creation of 1,600 jobs and potentially add £1.5bn to the UK economy by 2025.
It was made against the backdrop of local opposition on environmental grounds and furious debate over airport expansion in the South East.
While City Airport’s investment would allow for more flights, the Government is under pressure to make a decision on which major airport, between Heathrow and Gatwick, is to get a new runway.
The plans for the Docklands airport allow for an extended terminal, new aircraft taxiway and parking spaces for planes at what it calls “one of the capital’s fastest growing airports”.
The Chancellor, Philip Hammond, made it clear that the UK’s vote to leave the EU was a factor behind the decision.

EMERGENCY Emirates #EK652 from Dubai diverted to Mumbai due to smoke in cabin
Emirates flight #EK652 from Dubai to Male, with 309 people on board, made an emergency landing in Mumbai. The emergency landing was necessitated after smoke was observed on-board, according to a report by news agency ANI. The aircraft is still in Mumbai.
“The ATC declared full emergency at the MIAL (Mumbai International Airport Limited) at 1359 hrs after a Boeing 777 flight of Emirates (EK 652) from Dubai to Male had to make an emergency landing following detection of smoke in the aircraft,” said a Mumbai airport official.

NEWS easyJet to recruit cabin 1,200 new crew across Europe
easyJet has today announced that it is to recruit more than 1,200 new permanent and fixed-term cabin crew positions as the airline continues its growth.
The carrier, which flies more than 250 aircraft on over 820 routes in 31 countries, will offer its new cabin crew positions across all of its 27 European bases.
Around half of these positions will be based in the UK.
The news comes after easyJet’s announcement earlier this year that it was opening recruitment for more than 350 pilots and providing career opportunities for more than 150 of its current first officers to be promoted into captain positions.
easyJet currently employs over 5,500 cabin crew who are at the forefront of the airline’s operations, ensuring the highest safety standards are met and providing friendly service for easyJet’s 70 million passengers every year.
ALERT Fire at London Gatwick airport station
Southern Rail reported that there was a fault with the electric third rail at Gatwick and trains are at a standstill.
A spokesperson for National Rail said: “A fault with the electric third rail at Gatwick is causing delays of up to 20 minutes to journeys to and from the stations. This is expected to continue until 10:15.”

#OnThisDay in 2000, Air France Flight 4590 (Concorde) crashes in Gonesse, France
16 years ago today, Air France Flight 4590 (F-BTSC) was a scheduled international passenger flight from Charles de Gaulle Airport, France to John F. Kennedy International Airport, New York. Concorde was carrying 100 passengers and 9 crew members.

ACCIDENT
Most of the passengers on board were German. The flight was chartered by German company Peter Deilmann Cruises. The passengers were on their way to board the cruise ship MS Deutschland in New York City for a 16-day cruise to Manta, Ecuador.
The pilots on board Air France Flight 4590 were Captain Christian Marty, 54, First Officer Jean Marcot, 50 and Flight Engineer Gilles Jardinaud, 58.
Departure was delayed by about one hour because the crew had requested a replacement of the thrust reverser pneumatic motor of the no.2 engine. Also the rear bogie truck of the left hand main undercarriage was replaced.
When all 100 passengers had boarded, the plane taxied to runway 26R. Takeoff weight was calculated to be 186,9 tons, including 95 tons of fuel, which is one ton over the maximum takeoff weight.
At 14:42:17 the crew were cleared for takeoff from runway 26R. At 14:42:31 the captain commenced take off. At 14:42:54, the co-pilot called one hundred knots, then V1 nine seconds later.
A Japanese passenger inside an Air France Boeing 747 which had just landed from Japan, and was waiting to cross Concordes runway on its way back to the terminal. He happened to be taking photographs of Concordes take off and captured these horrific photos.
A few seconds after V1 the number 2 tyre (right front) on the left main landing gear was destroyed after having run over a strip of metal lost by a Continental Airlines DC-10-30 (N13067) which departed Paris as Flight 55 to Newark five minutes before.
The destruction of the tyre in all probability resulted in large pieces of rubber being thrown against the underside of the left wing and the rupture of a part of tank 5. A severe fire broke out under the left wing and around the same time engines 1 and 2 suffered a loss of thrust, severe for engine 2, slight for engine 1.
At 14:43:13, as captain commenced the rotation, the controller informed the crew the presence of flames behind the aircraft. The co-pilot acknowledged this transmission and the flight engineer announced the failure of engine no 2. Nine seconds later the engine alarm sounded and the flight engineer announced “shut down engine 2” then the captain called for the “engine fire” procedure. A few seconds later, the engine 2 fire handle was pulled and the fire alarm stopped. The co-pilot drew the captains attention to the airspeed, which was 200 knots.
At 14:43:30 the captain called for landing gear retraction. The controller confirmed the presence of large flame behind the aircraft. Twelve seconds later the engine fire alarm sounded again for around 12 seconds.
It sounded for the third time at about 14:43:58 and continued until the end of the flight.
At 14:43:56 the co-pilot commented that the landing gear had not retracted and made several callouts in relation to the airspeed. Three seconds later, the GPWS alarm sounded several times.
The co-pilot informed ATC that they were trying for Le Bourget aerodrome. Then the number 1 engine lost power as well. The aircraft entered a left turn until control was lost, crashing into hotel ‘Hotellisimo’ and bursting into flames. All 100 passengers & 9 crew members were killed as well as 4 people on the ground.
INVESTIGATION
The investigation was led by the BEA (Bureau of Enquiry and Analysis for Civil Aviation Safety). The BEA published their final report on Air France Flight 4590 on 16 January 2002. The investigation concluded that:
The aircraft was overloaded by 810 kilograms (1,790 lb) above the maximum safe takeoff weight. Any effect on takeoff performance from this excess weight was negligible.
After reaching takeoff speed, the tyre of the number 2 wheel was cut by a metal strip (a wear strip) lying on the runway, which had fallen from the thrust reverser cowling door of the number 3 engine of Continental Airlines DC-10. The wearstrip had been replaced at Tel Aviv, Israel, during a C check on 11 June 2000. Further maintenance work had been performed at Houston, Texas, but the strip had been neither manufactured nor installed in accordance with the procedures as defined by the manufacturer.
Aborting takeoff would have led to a high-speed runway excursion which would have led to the total loss of the aircraft.
The damage to the planes structure was so severe that the crash would have been inevitable, even with the engines operating normally.
The accident led to modification being made to Concorde, including more secure electrical controls, Kevlar lining to the fuel tanks and specially developed bursting-resistant tyres.
Just before the service resumed, the 11 September attacks took place, resulting in a marked drop in passenger numbers and contributing to the eventual end of Concorde flights. Air France stopped flights in May 2003, while British Airways ended its Concorde flights in October 2003.

NEWS Disgruntled Cathay Pacific passenger rips roof of new A350 looking for space
A Cathay Pacific plane bound for Bangkok, Thailand, was delayed from taking off from Hong Kong’s international airport after drama ensued during boarding.
An unidentified passenger from Hong Kong who was trying to find space to stow his luggage was displeased that configuration of the new A350 aircraft meant there weren’t any lockers above his seat.
Despite flight attendants telling him that they’d find storage space for him elsewhere, the passenger refused to believe them.
He then tries to open a panel but then rips the roof panel open with his bare hands, determined to find the space for his luggage. All he was met with was a gaping hole and shock from his fellow passengers.
NEWS Sunderland Football team to France makes emergency landing
Sunderland AFC were forced to make an emergency landing at Manchester Airport.
The squad, along with newly appointed boss David Moyes, had been on an Eastern Airways flight from Newcastle to Geneva when pilots noticed a warning light.
The aircraft was showing a technical failure, and it turned round between Stafford and Derby and diverted to Manchester.
A spokeswoman for Manchester Airport said the flight landed safely on the runway just after 11.20am.

NEWS Manchester United plane forced to make emergency landing due to bad weather
Manchester United plane was forced to make an emergency landing, leaving manager Jose Mourinho and his squad marooned somewhere in China.
Memphis Depay, the team’s winger, posted on his Facebook page that they were “lost” after adverse weather had forced their charter flight to land.
United had left Shanghai, where they were staying on the first leg of their pre-season tour of China, for Beijing. They are due to face Manchester City in the International Champions Cup at the National Stadium in the Chinese capital on Monday lunchtime.
ALERT Police investigate a bomb scare on #QFA737 at Queenstown airport
Police are still trying to find the passenger who left a note about a bomb on a plane in Queenstown. The airport was evacuated, although flights still took off after 3pm, when emergency services were alerted to the note. It was discovered by a cleaner after passengers had left the plane. The note contained information about a bomb, but a police search found there was no evidence of an explosive on board or in the terminal.

A British Airways flight packed with holidaymakers on their way from Gatwick to Crete had to make an emergency landing after the smell of “cannabis” filled the plane.
Flight BA2552 had to return to Gatwick after around an hour-and-a-half because of the pungent stench.

After a diversion over Paris, the plane was sent back to London where the 174 travellers on board had an hour-long wait for a replacement flight on Thursday morning, The Sun reported.
Fire crews were also sent on board to investigate the overpowering odour.
Hundreds of other travellers stuck in Crete by the ensuing disruption faced delays of up to six hours.
A British Airways spokeswoman said no evidence of cannabis or anybody smoking cannabis was found on board.
She added: “Our pilot returned the aircraft to Gatwick as a precaution following reports of an unidentified strong smell in the cabin.

The Irish Aviation Authority (IAA) wishes to advise the residents of Dublin City and its environs that the Italian Aerobatic Display Team, the Frecce Tricolori, will carry out a flypast along the River Liffey at approximately 15:15 today, 21st July, in preparation for landing at Casement Aerodrome, Baldonnell.
The Frecce Tricolori will be taking part in the Bray Air Display this coming weekend on both Saturday 23rd and Sunday 24th July.
For public information purposes the IAA wishes to advise residents that the aircraft will fly in formation of up to 13 aircraft. These aircraft are high powered jets, which will create an audible signature across Dublin during their flights.

NEWS Large wing piece in Australia to be tested for #MH370 link
A large piece of aircraft debris, believed to be a wing flap, has arrived in Australia for testing as part of the search for Malaysia Airlines flight MH370, officials said Monday.
The piece, found by locals on Pemba Island just off the coast of Tanzania on June 23, was now in Canberra, the Australian Transport Safety Bureau (ATSB) said.
“Technical specialists from the ATSB are working with Malaysian investigators to determine if it is from the Malaysia Airlines Boeing 777, MH370,” the bureau said in a statement.
The ATSB has so far examined four pieces of debris on behalf of Malaysia and determined they are almost certainly from MH370, a flight from Kuala Lumpur to Beijing which disappeared in March 2014 with 239 people on board.

Two pilots have been arrested for allegedly attempting to fly a transatlantic passenger plane from Glasgow Airport while under the influence of alcohol.
The captain and first officer are due to appear at Paisley Sheriff Court today following the incident on the Air Transat aircraft on Monday afternoon.
The cabin crew raised the alarm shortly before the plane’s scheduled 1pm take-off to Toronto.
A Police Scotland spokesman said: “Police can confirm two men aged 39 and 37 have been arrested in relation to allegedly being impaired through alcohol.”
A spokesman for the Canadian airline said: “We will await the results of the investigation and judicial proceedings before commenting on the matter before making any further comments. The safety of our crews and passengers is, and will remain, a top priority at Air Transat.”
A rescheduled flight is expected to depart on Tuesday.

A CRJ9 flight operated by Air Nostrum (Iberia Group) to Vigo (Spain) scheduled for departure at 15.10 from Fiumicino Airport, took off after 16:30 due to a “technical problem”.
No one knew how to push the aircraft on the runway. With all the passengers on board, about 15 people of Aviapartner were forced to push the plane on the runway.

NEWS pressurisation issue returns American Airlines flight to LaGuardia.
Travellers have had a bad start to their week with an American Airlines flight returning to LaGuardia due to a suspected pressurisation issue.
The American Airlines flight AA4595 that was due into Columbus at 8:00am from LaGuardia was diverted back to New York.
Unconfirmed reports suggest the diversion was caused due to a malfunction is the aircraft’s pressurisation system.
The flight took off from LaGuardia Airport 6:40am but the flight was interupted at 11,000 feet when the crew noticed the issue.
The crew elected to squawk the general emergency code 7700 and safely landed back at LaGuardia at 8:00am.
The Embraer 170 was carrying 12,000 pounds of fuel and 70 souls were onboard at the time of the incident.
No injuries have yet been reported and the aircraft didn’t require assistance upon landing.
NEWS An easyJet passenger urinated on others on board flight #U26984
Passengers on an Edinburgh flight were left drenched when a man urinated on some of them as they waited to get off a flight.
The 26-year-old alleged urinator was on board a flight from Copenhagen to Edinburgh and tried to leave the aircraft just after it landed.
Flight attendants asked the unnamed man to sit down – and his response was to begin peeing on people around him.
Police were called to attend at the incident.
A spokeswoman for EasyJet said: “easyJet can confirm that flight EZY6984 from Copenhagen to Edinburgh on 13 July was met by police on arrival in Edinburgh as a result of a passenger onboard behaving in a disruptive manner.

20 years ago today, Trans World Airlines Flight 800 (N93119) was a scheduled international passenger flight from John F Kennedy International Airport, New York City to Charles de Gaulle Airport, Paris with a stop over at Leonardo da Vinci Airport, Rome. The Boeing 747-131 was carrying 212 passengers & 18 crew members.

ACCIDENT
The pilots on board TWA Flight 800 were Captain Ralph G. Kevorkian, Co-pilot Steven E. Snyder and Flight Engineer Richard G. Campbell. All had more than 30 years employment with Trans World Airlines. There was also a Flight Engineer Trainee on board Oliver Krick, who was starting the sixth leg of his initial operating experience training.
The TWA Boeing 747 (N93119) arrived as TWA Flight 881 from Athens, Greece at John F Kennedy International Airport, New York at 16:31. The airplane was refuelled at JFK and remained at gate 27 with the APU and two of its three air conditioning packs operating for about 2 1/2 hours until it departed as TWA Flight 800.
TWA Flight 800 was scheduled to depart JFK for Paris at about 19:00. However, the flight was delayed because of a disabled piece of ground equipment and concerns about a suspected passenger/ baggage mismatch. After the owner of the baggage in question was confirmed to be on board. The flight crew prepared for departure and was pushed back from the gate at 20:02.
At 20:18, ATC advised the pilots that the wind was out of 240-degrees at 9 knots and cleared TWA Flight 800 for takeoff. After takeoff the pilots received a series of altitude assignments and heading changes from New York Terminal Radar Approach Control and Boston ARTCC controllers.
At 20:25, Boston ARTCC advised the pilots to climb and maintain FL190 and expedite through FL150. At 20:26, Boston ARTCC amended TWA Flight 800’s altitude clearance, advising the pilots to maintain FL130.
At 20:29, the captain stated, “Look at the crazy fuel flow indicator there on number four…. see that?” One minute later Boston ARTCC advised them to climb and maintain FL150. The crew then selected climb thrust.
A loud sound was heard for a fraction of a second then the CVR stopped recording at 20:31.
At that moment, the captain of an Eastwind Airlines Boeing 737 reported to Boston ARTCC that he “just saw an explosion out here”, adding “we just saw an explosion up ahead of us here…. about 16,000ft or something like that, it just went down into the water”.
The aircraft broke up and debris fell into the Atlantic Ocean 8 miles south off East Moriches, New York. All 212 passengers & 18 crew members were killed.
INVESTIGATION
The Investigation was led by the National Transportation Safety Board (NTSB). The government spent four years and millions of dollars in the investigation.
Investigators considered several possible causes for the structural breakup: structural failure and decompression, detonation of a high-energy explosive device, such as a missile warhead exploding either upon impact with the airplane or just before impact, a bomb exploding inside the airplane or a fuel/air explosion in the center wing fuel tank.
It was eventually concluded that an explosion of the center wing fuel tank (CWT) resulted from ignition of the flammable fuel/air mixture in the tank. The source of ignition energy for the explosion could not be determined with certainly but, of the sources evaluated by investigation, the most likely was a short circuit outside of the CWT that allowed excessive voltage to enter it through electrical wiring associated with the fuel quantity indication system.
Contributing factors to the accident were the design of certification concept that fuel tank explosions could be prevented solely by precluding all ignition sources and the design and certification of the Boeing 747 with heat sources, located beneath the CWT with no means to reduce the heat transferred into the CWT or to render the fuel vapor in the tank nonflammable.

NEWS Alitalia #AZ827 to Rome was delayed 16h after pilot claimed he was too tired to fly the plane
Alitali flight AZ827 Airbus A321 (reg. EI-IXV) from Beirut to Rome was delayed approximately 16 hours.
Passengers did board the plane but the pilot didn’t want to takeoff. He said that he was too tired to fly the plane back to Rome.
ALERT Several planes collided at Minsk Airport due to a storm
Several planes have been damaged by a storm at the airport in the capital of Belarus, with two aircraft colliding on the airfield. There have been no reports of injuries.
The planes that collided belong to the Belarusian Belavia Airlines and the Ruby Star Air Carrier Company. There were no passengers on board.
The airport press service said that several windows in the airport terminal had been smashed and a suspended ceiling system was deformed in several places due to rough wind and heavy rains.
Several flights were also re-routed to alternate airport in the Belorussian city of Gomel, and another was re-routed to the city of Vilnius in neighboring Lithuania.
EMERGENCY Air Canada #AC7212 nose gear collapsed on landing at Calgary airport
Air Canada Express flight AC7212 landed safely at around 7:20 a.m., a spokesperson for Air Canada said the “nose gear collapsed when the aircraft came to a stop.”
“We recognize that this would have been disconcerting to passengers, and can confirm that our regional Express partners are trained to handle events such as this and to look after our customers with care,” Angela Mah said in an email to Global News.
The flight, operated by Air Georgian, was travelling from Lethbridge to Calgary.
According to Mah, Air Georgian is inspecting the aircraft and will be “taking care of its maintenance as required.”
Fifteen people were on board the plane, a Beechcraft 1900 aircraft.

A tweet posted at 6.46pm to American Airlines read: ‘One of those lovely Boeing airplanes has a tick, tick, ticking in it. Hurry gentlemen, the clock is ticking.’
Another, sent 6 minutes earlier to Delta Air Lines, read: ‘There’s a nice tick, tick in one of those lovely Boeing planes, high quality.’
The judge warned the boy, who has no previous convictions, that he could receive a custodial sentence when he is dealt with later this month. ‘You may be a young man but you are a clever young man’ she told the teenager, who sat next to his mother in the court.

BREAKING United flight #UA1594 surrounded by fire crew at Bush Airport in Houston
A flight reportedly leaving for Denver suffered an apparent technical issue on takeoff from Bush Airport in Houston early Monday.
FlightAware.com says United flight 1594 was supposed to take off shortly after 6:30 a.m.
The flight was taxiing when it was surrounded by firefighters and other emergency personnel.
A ladder was put in place, but it did not look like any passengers got off the plane.

#OnThisDay in 1991, Nigeria Flight 2120 suffers a catastrophic in-flight fire over Jeddah
25 years ago today, Nigeria Airways Flight 2120 (C-GMXQ) was a chartered passenger flight operated by Nationair from King Abdulaziz International Airport, Jeddah, Saudi Arabia to Sadiq Abubakar III International Airport, Sokoto, Nigeria. The Douglas DC-8-61 was carrying 247 passengers & 14 crew members.

The pilots on board Nigeria Airways Flight 2120 were Captain William Allen, 47 a former Canadian Air Force pilot, had logged 10,700 flight hours. First Officer Kent Davidge, 36, had logged 8,000 flight hours and Flight Engineer Victor Fehr, 46, had logged 7,500 flight hours.
Nigeria Airways Flight 2120 started its take off roll from Jeddah’s runway 34L at 08:28, bound for Sokoto, Nigeria. At about 15 seconds into the take off, an oscillating sound was heard in the cockpit. Within two seconds, the flight engineer said: “What’s that?” The first officer replied: “We gotta flat tire, you figure?” Two seconds later, an oscillating sound was again heard. The captain asked the first officer: “You’re not leaning on the brakes, eh?” The first officer responded: “No, I’m not, I got my feet on the bottom of the rudder.”
At that time, the aircraft had accelerated to about 80 knots. Marks on the runway showed that the No.1 wheel started to break up at about this time. In addition, the left and right flanges of No.2 wheel began to trace on the runway; rubber deposit from No.2 tire continued which appeared to be from a deflated tire between the flanges.
At 28 seconds into the take off, the captain called 90 knots this was acknowledged by the first officer. The captain called V1 at about 45 seconds. Two seconds later, the first officer noted: “sort of a shimmy like if you’re riding on one of those ah thingamajigs.”
At 51 seconds, the captain called rotate and the airplane lifted off the runway. Witnesses noticed flames in the area of the left main landing gear. The flames disappeared when the undercarriage was retracted.
During the next three minutes several indications of system anomalies occurred, which included pressurisation system failure, a gear unsafe light and a loss of hydraulics.
The captain requested a level-off at 2000 feet because of the pressurisation problem. In his radio call the captain used the callsign “Nationair 2120” instead of “Nigerian 2120”. Confusion set in for the air traffic controller and mistook the transmission to be from a Saudi flight returning to Jaddah and cleared the Jeddah bound aircraft to 3000 feet. The captain of Nigerian 2120 acknowledged the ATC transmission without a call sign, saying “understand you want us up to 3000 feet.”
This misunderstanding continued for the next three minutes with ATC assuming that all calls were from the Saudi flight not from Nigerian Airways Flight 2120. About a minute later the captain reported that the aircraft was leveling off at 3000 feet. The first officer interrupted with “… declaring an emergency. We’re declaring an emergency at this time. We believe we have ah, blown tires.”
As the aircraft continued on the downwind heading, a flight attendant came into the cockpit and reported “smoke in the back…. real bad.” A few moments later, the first officer said “I’ve got no ailerons.” The captain responded “Ok hang on, I’ve got it”.
This was the last record on the CVR which failed along with the FDR at 08:33:33.
The ATC controller gave a heading to intercept the final approach and thereafter continued to give heading information.
Meanwhile, during the downwind and base leg, the fire had consumed the cabin floor above the wheel wells, permitting cabin furnishing to sag into the wheel wells. When the gear was probably extended at 11 miles on the final approach, the first body fell out because fire had burned through the seat harness. Subsequently, with the gear down and forceful air supply through the open gear doors, rapid destruction of more floors structure permitted the loss of more bodies and seat assemblies. Despite the considerable destruction to the air frame, the aircraft appeared to be controllable.
Eight minutes after the aircraft began its take off and 10 miles from the runway, the captain declared an emergency for the third time saying. “Nigeria 2120 declaring an emergency, we are on fire, we are on fire, we are returning to base immediately.
The aircraft came in nose down and crashed 9,433 feet short of the runway at 08:38. All 247 passengers & 14 crew members were killed instantly. The crash of Nigeria Airways remains the deadliest accident involving a DC-8 & the deadliest aviation disaster involving a Canadian airline.
INVESTIGATION
The Investigation was led by the Canadian Transportation Safety Board. They concluded that prior to departure, the lead mechanic had noticed that the #2 and #4 type pressures were below the minimums for flight dispatch and attempted to inflate them, but no nitrogen gas was readily available, and the project manager, unwilling to accept a delay, disregarded the problem and readied the aircraft for dispatch.
As the aircraft was taxiing, the transfer of the load from the under-inflated No.2 tire to the No.1 tire on the same port side axle resulted “in over deflection, over-heating and structural weakening of the No. 1 tyre. “The No.1 tyre failed very early on the take off roll”, followed almost immediately by the No.2. The latter stopped rotating “for reasons not established”, and the subsequent friction of the wheel assembly with the runway generated sufficient heat to start a self-sustaining fire.
The crew realised there was a problem, but not the nature or seriousness of it. The aircraft was not equipped with fire or heat sensors in the wheel assembly. When the landing gear was retracted. “Burning rubber was brought into close proximity with hydraulic and electrical system components”, causing the failure of both hydraulic and pressurisation systems that led to structural damage and loss of control of the aircraft.
